Social Worker – Health Care Center
The Portage County Health Care Center has a great opportunity to join our 5 STAR FACILITY. We are hiring a full time Social Worker to join our team!
Summary of Position:
This position provides a full range of Social Service functions in the areas of conducting admissions, discharge planning, assisting residents to attain and maintain the highest quality of life possible, meeting each resident’s psychosocial needs, and by taking the position of advocate to assure the resident rights are maintained. This position also involves assisting the Administrator, Director of Nursing and Clinical Nurse Liaison with the planning, developing, organizing, implementation, and evaluation of the Social Service Department in accordance with the current existing Federal and State standards, as well as Portage County and Portage County Health Care Center established policies and procedures.
What are some of the primary job responsibilities?
Admissions:
Coordinate admissions with Interdisciplinary Team to best meet resident needs and work with area hospitals to provide appropriate placement options.
Complete PASARR (Preadmission Screen and Resident Review) form OBRA (Omnibus Budget Reconciliation Act) prior to or at the time of admission to determine appropriateness of placement and/or whether a Level II screen is necessary; refer to Maximus for Level II screen when appropriate.
Provide facility tours for prospective residents, families, or interested parties as needed.
Discharge Planning:
Determine and coordinate discharge plan with Interdisciplinary Team for each resident in conjunction with the resident and/or legal representative upon admission and review quarterly and/or annually based on resident’s wishes and desires.
Assist with determining needs and make referrals to appropriate agencies/community resources prior to discharge and assist in coordinating appropriate supportive services to address gaps in needs to ensure continuity of care.
Assist Residents in Attaining and Maintaining Their Highest Quality of Life Through Meeting Their Psychosocial Needs and Assuring That Their Personal and Medical Needs Are Met:
Develop and maintain a good working rapport with residents, family members, legal representatives, PCHCC staff, area hospitals and agencies to assure that the Social Service programs/interventions can be properly maintained to meet the needs of all residents.
Assist with the planning, developing, organizing, and implementing of Social Service programs, policies, and procedures.
Keep abreast of current Federal and State regulations as well as maintain professional standards.
Refer to other professional counselors when indicated.
Provide 1:1 time with residents and families to address concerns as needed.
What kind of experience and education is needed?
Bachelor’s degree in Social Work or related field from an accredited University.
Certified Social Worker in the State of Wisconsin preferred. Certification required within one year of employment start date.
Minimum of one-year experience as a Social Worker in a nursing home is highly desired.
