The positions include:

Social Worker/Case Manager- Adult Protection
-Our Adult Protective Services unit is looking for composed, critical thinking and team-oriented individuals to join our team now! Provides assessment and crisis intervention services; responsible for the investigation of persons referred under the Adult at Risk and the Elder Abuse Neglect reporting systems; determines needs and develops short term intervention and support plans; manages services; coordinates and petitions the court for Guardianship Services; and refers consumers to other appropriate service providers.

Social Worker/Case Manager- CLTS
-Our CLTS unit is looking for highly organized, accountable and team-oriented individuals to join our team now!
Provides case management through CLTS Support and Service Coordination to children who are diagnosed with physical, emotional or cognitive disabilities. The goal of the CLTS Waiver Program is to support children with substantial needs, as well as their parents/guardians, by delivering services to promote the goal of every child living their best possible life now and into the future.

Social Worker/Case Manager- Community Mental Health Program
-Under supervision of Behavioral Health Supervisor provides referral, case management, and direct services to adults with chronic mental illness and/or substance use disorders.

Social Worker/Case Manager- CPS (Family Recovery Court)
-Family Recovery Court is a treatment court that incorporates the traditional treatment court model along with the family centered child protection standards for practice. The Family Recover Court works in conjunction with Child Protection to support timely recovery of parent(s) from substance dependence to enhance the overall function of the family unit as a whole and to improve the safety, well-being, and permanence for children. Services to the participates are provided by the guidance of the multidisciplinary treatment court team which includes a Judge, Legal Partners, Probation, Community Providers, Health and Human Services, and Law Enforcement. The Family Recovery Court Case Manager is responsible for monitoring the participants in compliance with the court and working with the CPS Case Manager on the ongoing case plan.

Social Worker/Case Manager- CPS Intake and Ongoing
-Responsible for intake, ongoing and foster care cases providing treatment intervention and case management services to families involved in alleged or adjudicated abuse; and some responsibility of investigating referrals of physical abuse, neglect, sexual abuse and emotional damage to children.

Social Worker/Case Manager-Homeless Outreach Team Lead
-This advanced level lead position will perform all duties of the social worker/case manager position, and in conjunction with the Behavioral Health Supervisor, assist in providing day to day direction and task assignments. This position will act as a lead worker in the areas of outreach and navigation to individuals and families who are homeless and at risk of entering other systems of care such as mental health, substance use, child welfare and youth justice.

Social Worker/Case Manager-Volunteer Services (Children, Youth and Families)
-Recruit, plan and train volunteers who are interested in mentoring children referred from the child welfare system.

Social Worker/Case Manager: CCS/CLTS Youth Service Facilitator
-Provides case management to children and adolescents who are diagnosed with mental health and/or substance use disorder diagnoses, including children with severe, emotional disturbance, pervasive mental illness and/or physical, emotional, or cognitive disabilities. The goal of the CCS program is to serve children and adolescents with mental health and/or substance abuse disorders who need ongoing services due to impairments that interfere with or limit healthy functioning. The CCS Service Facilitator’s primary responsibility will be in the Comprehensive Community Services program, while serving children dually eligible for Children’s Long-Term Support Waivers.

Social Worker/Case Manager: Criminal Justice Services; Report Center
-Provides pretrial services, including intake, assessment, case management, and direct services to participants/prospective participants who have been identified through the Criminal Justice system. Services provided will be under the guidance of the pretrial services team and directed by the Pretrial Services Supervisor.
