The Clinical Services Unit Social Worker/Case Manager provides case management and support services to individuals affected by mental health and substance use challenges, with a primary focus on serving individuals under Chapter 51 Commitment Orders. Key responsibilities include outreach, assessments, developing and implementing case plans, coordinating treatment and support services, providing crisis intervention, and maintaining accurate client records. This position also requires a working knowledge of the legal rights of individuals receiving mental health and substance use services and collaboration with community partners and service providers.
Applicants must have a degree in Social Work, Human Services, Psychology, Nursing, or a closely related field. Certification or eligibility for certification as a Social Worker or Registered Nurse in Wisconsin is preferred. Two years of experience providing services to individuals with mental health and/or substance use disorders is preferred. A valid driver’s license is required.
This position may be allowed remote work for a portion of the work schedule, depending on the needs of the position, organization, work unit requirements and employee’s preference.
