Wage: $76,315.20 to $86,340.80/annually
Purpose of Classification
The purpose of positions in this classification is to advocate on behalf of patients, assess patient needs, develop and provide therapy in a group or an individual setting, develop care/treatment plans and discharge plans in an acute care psychiatric hospital. Rotating weekends and holidays are required.
Essential Duties and Responsibilities
The following duties are normal for this classification. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
1. Complete patient intakes to include, but not limited to: social assessments, Columbia Suicide Severity Rating Scale, initial care plans, obtain collateral from outpatient providers and/or social supports, and guardian consents.
2. Maintain communication with human service agencies and county courts regarding patient legal status, diagnoses, progress, and to coordinate discharge planning and hold meetings as necessary.
3. Acts as liaison to psychiatrist, Corporation Counsel regarding patient legal status, and family members.
4. Meet with assigned case load 1:1 daily to complete therapeutic activities and case management tasks and maintain documentation through daily progress notes.
5. Conduct therapy groups.
6. Assist psychiatrist with substantiating diagnoses during treatment plan meetings, update substantiated diagnoses in the Master Treatment plan and subsequent treatment plan updates.
7. Complete discharge process to include, but not limited to: aftercare with follow-up appointments, ASAM to determine appropriate level of care, referrals to community resources, placement referrals, safety plans, family sessions, transfers to other facilities, C-SSRS, Medicare forms, and sending records to appropriate parties.
8. Maintain documentation.
9. Maintains knowledge of human behavior; acute and chronic mental illness treatment strategies, treatment and intervention for persons with developmental disabilities; and current social work principles, practices and theory. Attends mandatory training and continuing education courses.
10. Responsible to know and practice the safety policies of Wood County. Perform all job tasks in a safe and prescribed manner.
Additional Tasks and Responsibilities
While the following tasks are necessary for the work of the unit, they are not an essential part of the purpose of this classification and may also be performed by other unit members.
1. Serves patients with different court documentation pertaining to their case throughout hospitalization.
2. May sit in on patient rounds with the psychiatry to help aid in the telehealth process.
3. Cover insurance authorizations as assigned.
EXPERIENCE, TRAINING, QUALIFICATIONS
Master’s degree in Social Work, one to two years social work experience preferred in an acute care psychiatric hospital, or any combination of education and experience that provides equivalent knowledge, skills and abilities. Possession of, or eligibility for, Social Work Certification in accordance with Wisconsin Chapter 457.08 is required.
The 3,000 clinical hours can be completed during employment in order to obtain clinical licensure.
