The Monroe County Rolling Hills Senior Living Community is hiring a full-time RCAC Manager/Social Worker. Successful candidates will have a Bachelor’s degree in social work or an approved equivalent Bachelor’s degree in a Human Services related program from an accredited university. A current Social Work certification with the state of Wisconsin or eligibility for certification is required and five years social work or related experience in a health care or long-term care setting is preferred. A minimum of three years supervisory experience is desired.

Our RCAC Manager/Social Worker serves in a dual position and is responsible to assist in the overall leadership, planning, administration and management of the Residential Care Apartment Complex (RCAC) and serves as a Social Worker in the nursing home.
– RCAC Manager: Directs the work of and manages direct care staff. Communicates with tenants, families, medical providers and AL nurse to coordinate care and develop individual service plans. Conducts tours and manages admissions and discharges.
– Social Worker: Works cooperatively with the Social Services Manager. Promotes and protects rights of residents and supports resident psychosocial well-being, quality of life and care. Coordinates resident admissions; conducts assessments of cognition, mood, behavior and discharge needs; and completes MDS assessments, plans of care, discharge planning and progress notes to comply with applicable policies, rules and regulations for assigned caseload.
