The Public Health Strategist – Home Visitation delivers case management services through the Bright Family Beginnings program, supporting families from prenatal care through children age five. In this role you will manage a complex caseload, advance health equity, promote individual and community well-being, educate caregivers, and empower families. The Strategist applies a multi-sector approach to strengthen departmental, community, and interagency partnerships that address Kenosha County’s health and equity needs. This position reports to the Public Health Nurse Supervisor – Bright Family Beginnings.
This grant-funded position is supported by a state grant through Family Foundations, has a current duration through September 2026, and is contingent on continued funding.

Position Summary and Job Duties
Grant Requirements
• Provide screenings virtually and via home visits to women and their families eligible for evidence-based programs
• Educate and provide guidance to parents on implementation of proper developmental activities for their children
• Adhere to the home visitation process and the fidelity of model program(s)
• Carry a maximum caseload of up to 20 families as defined by model program(s)
• Plan, coordinate, and conduct grant activities outlined in the work plan and grant specifications
• Develop and maintain cooperative working relationship with external agencies and organizations such as fire departments, emergency departments, law enforcement agencies and behavioral health organizations
• Provide support and education to internal and external partners in area of expertise, partnering to implement programs
• Develop information, resources, and reports including work plans
• Identify populations negatively affected by events and crisis and the conditions, systems and structures that support these outcomes
• Assist in the development of information and resources for community education
• Collaborate and participate in local community organizational meetings to develop relationships with new and existing members and seeking out new sources of funding
• Create relevant presentations to deliver to community-based organizations, local/state/national stakeholders at conferences, and members of Kenosha County Public Health
• Assist clients with establishing goals and outcomes with educational supports
Quality Improvement
• Continually seek out quality improvement opportunities to refine/enhance the diversity, equity, and inclusion of programs and services
• Assist with identifying, developing, and advancing public health interventions to improve social determinants of health and health equity in our community
• Assist in leading division’s community health planning initiatives to promote individual and population health in program area
• Participate in broader divisional orientation, training, and professional development workshops/education including the Community Health Improvement Plan (CHIP) and Strategic Plan
• Participate in weekly team meetings to receive updates on upcoming changes, new implementation protocols, and important internal issues that require immediate attention
• Review scientific evidence related to public health issues, concerns, interventions, and evidence based or evidence-informed practices
Data Collection, Analysis, & Reporting
• Collect data from a variety of sources and enter multiple software systems
• Implement data collection methods and tools to evaluate the effectiveness of programs and services including working with partners for improved data sharing
• Identify and define populations of interest to develop health studies for specific target populations and programs for the community
• Adhere to the fidelity of model programs

Job Requirements, Education, Training and Experience
Required Education and Experience
• Bachelor’s degree in public health, public policy, health education or related field
Preferred Education and Experience
• Master’s degree in public health, public policy, health education or related field
• 2+ years of experience in community health, coalition coordination, program coordination, and/or functions related to the position
• Bilingual – English and Spanish
• Training in program area of mental health first aid/trauma informed care
Other Job Requirements
• CPR certification obtained within 90 days of hire
• Valid driver’s license and acceptable driving record
• Access to a vehicle with active automobile insurance for use in the performance of the job (mileage reimbursement provided)
• Availability to perform occasional weekend and evening work as required
• Availability to perform other work as required or assigned including, but not limited to, a 24/7 response during public health emergency situations and disease outbreak investigations
