PURPOSE OF POSITION:
The purpose of this position is to promote community health and wellness by focusing on community engagement, partnership development, and encouraging collective action to reduce health outcome disparities. The Public Health Planner works directly with Director/Health Officer to connect with department partners and clients to seek out new projects, engage the community in the department’s work, and provide expert advice.

ESSENTIAL FUNCTIONS:
Develop and implement the Trempealeau County Health Department’s community health assessment (CHA) and administer the public health emergency preparedness (PHEP) planning program.

Develop and implement a written CHIP every three years by assessing applicable data, developing measurable health objectives, and partnering with persons, agencies, and organizations to cultivate community ownership throughout the entire development and implementation of the plan.

Implement data collection methods and tools to evaluate the effectiveness of CHIP activities. Report CHIP activities, events, and updates to internal Trempealeau County staff and community stakeholders through consistent communication.

Serve as the Public Information Officer (PIO) with all relative Incident Command System (ICS) – PIO Training. Serve as marketing manager working as the main point of contact for media as well as developing and executing the marketing plan for the health department. Design and develop promotional materials for all departments within TCHD, including flyers, posters, brochures, etc.

Coordinate with Great Rivers United way to ensure activities for the Compass Report are planned and executed every three years.

Lead projects related to emerging public health issues focused on systems and policies development to improve socioeconomic determinants of health and reduce health disparities.

Create, convene, support, and sustain strategic, non-program specific relationships with key community groups or organizations representing populations experiencing health disparities or inequities; private businesses and health care organizations; relevant federal, Tribal, state, and local government agencies; elected and non-elected officials.

Establish and maintain trust with and authentically engage community members and populations most impacted by inequities in key public health decision-making and use community-driven approaches; Utilize community input to advise the Director/Health Officer on developing public health policies and programs.

Coordinate developing the department’s community engagement strategy on public health policies, programs, and resources.

Arrange the department’s presence at community events and ensure the representatives have appropriate outreach materials.

Advance partnerships with community organizations with the purpose of elevating marginalized community voices in decision-making processes and public health planning. Engaging community members as experts and key partners.

Engage community members in assessing, implementing, monitoring, evaluating, and modifying community health planning, including engaging diverse populations and considering adversely impacted populations.

Provide public health policy analysis with particular emphasis on the social determinants of health and achieving greater health equity.

Identify and champion grant opportunities within the organization, and lead in writing, developing the budgets, and building new or existing projects through the grants.

Collaborate and facilitate data sharing with partners, including multi-sector partners. Analyze and use disaggregated data to track issues and inform equitable action.

Convene and facilitate multi-sector partnerships and coalitions that include sectors that influence health.

Foster and build genuine, strength-based relationships with a diverse group of partners that reflect the community and the population. Authentically engage with community members and organizations to develop public health solutions. Learn from and support, existing community partnerships and contributing public health expertise.

Represent the department on local, regional, and statewide groups connecting with emergency preparedness. Participate in regional Healthcare Emergency Readiness Coalition (HERC) activities and meetings. Serve on the Western Region Public Health Readiness Consortium.

Coordinate and plan public health preparedness efforts related to developing, implementing, exercising and evaluating action plans. Ensure training of department staff for emergency response roles. Work closely with health department staff to ensure public health emergencies are responded to effectively.

Respond to public health emergencies. Lead outbreak investigation coordination with other health department staff. Use real time data to identify and respond to acute outbreaks, emergencies, and other health hazards.

Ensure that all PHEP grant deliverables are negotiated, budgeted, met, and reported annually. Actively manage the Wisconsin Emergency Assistance Volunteer Registry. Establish and promote basic, ongoing community readiness, resilience, and preparedness by enabling the public to take necessary action before, during, or after a disaster, emergency, or public health event.

Maintain ongoing relations with local and statewide media including the ability to write a press release, conduct a press conference, and use electronic communication tools to interact with the media.

Coordinate the development of media campaigns to address public health issues. Inform the public on developing urgent and emerging public health issues through press releases, public statements, radio addresses, newspaper ads, and relevant social media posts.

Manage social media accounts, including Facebook, YouTube, and LinkedIn. Coordinate with the Trempealeau County IT department to maintain the Trempealeau County Health Department (TCHD) website.

Prepare and provide public presentations on the public health role, strategy, and impact associated with public health issues. Provide accurate, timely, and understandable information, recommendations, and instructions to the public during a disaster, outbreak, or emergency.

Develop and implement a proactive health education/health communication strategy (distinct from risk communication) that disseminates timely and accurate information to the public designed to encourage actions to promote health in culturally and linguistically appropriate formats for the various communities served, including using electronic communication tools.

Communicate with accuracy and necessary speed using appropriate communication channels to effectively reach the intended populations. Develop and deploy culturally and linguistically appropriate and relevant communications and educational resources, which includes working with stakeholders and influencers in the community to create effective and culturally resonant materials.

Employ the principles of risk communication, health literacy, and health education to inform the public when appropriate. Actively engage in two-way communication to build trust with populations served and ensure accuracy and effectiveness of prevention and health promotion strategies.

Initiate, plan, execute, monitor, and close high profile, complicated, and/or new projects.

Coordinate training, continuing education, and dissemination of information and resources to Community Health Workers (CHWs) and American Health Education Center (AHEC) interns. Monitors program progress, goals, and objectives by maintaining regular contact with CHWs and/or AHEC interns.

Coordinates project planning for maternal and child health, CHIP, and other strategic priority program initiatives. Possible projects include Covid response, overdose response, mental health, health equity, rural safety, etc.

Assist other health department programs in developing service delivery feedback strategies. Work with health department staff to ensure alignment of strategic priorities and assist in program planning.

KNOWLEDGE, SKILLS, AND ABILITIES:
Attention to detail and strong communication skills.

Ability to effectively communicate orally and in writing with public health and healthcare professionals and internal partners.

Knowledge of the Health Insurance Portability and Accountability Act (HIPAA) requirements and the importance of protecting health information.

Ability to complete assignments in a timely and efficient manner.

Ability to prioritize and manage multiple tasks and work independently.

Demonstrated reliability and integrity.

Bilingual (English/Spanish), or experience working with an interpreter preferred.

TRAINING, EXPERIENCE AND OTHER REQUIREMENTS:
Bachelor of Science degree preferably in Public Health, Health Education, Public Administration, Non-Profit Management, Public Health Programing, and two years of experience in managing projects of high complexity and a variety of internal/external stakeholders, or any combination of education and experience that provides equivalent knowledge skills and abilities. Specific experience in the areas of community health, coalition coordination, program coordination and/or related functions are vital. Previous experience using Microsoft Office, social media platforms (Facebook, LinkedIn, etc.), CANVA, Lucid Chart, and Jotform preferred. Valid motor vehicle operator’s license required.
