POSITION SUMMARY:
St. Croix County Public Health is seeking a qualified nursing professional to provide public health nursing services, education, disease prevention, and community health support services to individuals, families, and populations within the community.

Depending on qualifications, education, licensure, and experience, the selected candidate may be hired as either a Public Health Nurse or Registered Nurse – Public Health.

ESSENTIAL FUNCTIONS:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned.

Core Duties – Performed by Both Positions

– Maintains current knowledge on medical information that influences services provided to clients.
– Assists with and/or manages communicable disease cases and outbreaks; investigates and educates regarding prevention and control in accordance with state mandates and statutes and maintains WEDSS (Wisconsin Electronic Disease Surveillance System) documentation.
– Assists in planning, preparing, and responding to Public Health preparedness events, including mass clinics.
– Provides direct client services following current department, state and federal guidelines and policies: Assesses, educates and evaluates client needs (i.e: pregnant and postpartum women, infants, reproductive health clients, tuberculosis program clients, immunization clients, general public health clients) via home visits, clinic/office visits, or phone calls; develops nursing care plans and coordinates care.
– Assesses, educates, and evaluates clients regarding acute and chronic health conditions, psychosocial, and environmental needs and refers to appropriate resources.
– Prepares, updates, and maintains client records, utilizing various electronic health record systems.
– Assists in organizing, managing, updating, and implementing various Public Health programs and contracts. – Coordinate projects to assure goals are addressed as assigned; Collect data and assist in analysis and outcome evaluation throughout all public health services and activities as appropriate.
– Assists in the writing of grant proposals, and oversees grant activities, reporting requirements, contractual obligations, and accreditation performance standards/requirements.
– Supports and participates in re-accreditation, 140 review, strategic planning, the performance management system including quality improvement, workforce development, emergency preparedness, community health needs assessment, and community health improvement planning as assigned.
– Provide appropriate health promotion, disease and injury prevention education in various formats to diverse audiences, including schools, media, organizations, and the public.
– Facilitates and manages community coalitions, networks and partnerships through outreach and collaboration.
– Works cooperatively and communicates effectively with local, county, state and federal partners, and inter-agency professional teams.
– Comprehensively assess needs related to public inquiries and provide appropriate education and referrals according to client needs.
– Other duties as assigned.

Additional Duties – Public Health Nurse (BSN Level)
The following duties may be assigned to employees hired into the Public Health Nurse classification:

– Provides leadership in planning, preparing, and responding to Public Health preparedness events, including mass clinics.
Organizes, manages, updates, and oversees various Public Health programs and contracts with local, state and federal partners.
– Provides leadership in communicable disease response activities, outbreak management, and public health program and policy implementation and assessment.
– Oversees grant activities, reporting requirements, contractual obligations, and accreditation performance standards/requirements. Supervises the health care services provided by LPN’s, CNA’s, CMA’s, and support staff related to patient care and community programs.
– Provides oversight of clinical educational experiences for students.
– May assist with planning, implementing, and evaluating environmental health programs.

MINIMUM QUALIFICATIONS:

EDUCATION AND EXPERIENCE REQUIREMENTS

Registered Nurse – Public Health

– Associates degree from a nursing program accredited by the National Professional Nursing Accrediting Organization.
– Wisconsin Registered Nurse license required.
– CPR Certification required.
– One year experience in health care or public health preferred.
– Fluency in Spanish preferred.
– Must successfully pass criminal and caregiver background check.

Public Health Nurse

– Bachelor’s degree from a nursing program accredited by the National Professional Nursing Accrediting Organization.
– Wisconsin Registered Nurse license required.
– CPR Certification required.
– Two or more years’ experience in public health nursing.
– Fluency in Spanish preferred.
– Must successfully pass criminal and caregiver background check.

Registered Nurse – Public Health:

Expected Pay Range: $67,184 – $76,003/year
Full Pay Range: $67,184 – $93,600/year

Public Health Nurse:

Expected Pay Range: $70,554 – $79,830/year
Full Pay Range: $70,554 – $98,301/year
