**In addition to hourly pay, this position will receive $2.50 per hour in lieu of fringe pay.**

Primary Responsibilities
The following duties are primarily performed and are essential for this position. Employees are expected to be able to perform each of these job duties satisfactorily and successfully to be qualified for the position. Other duties may be required and assigned.

Provide formal and informal evaluations assessing functional capabilities of infants and toddlers on assigned caseload.
Develop and implement specific goals with measurable outcomes in the Individual Family Service Plan in conjunction with the IFSP team.
Maintain all Early Intervention documentation including EI state forms and written documentation of all evaluations, goals, physical therapy treatment plans, progress notes, and communication with doctors and families, in accordance with agency formats in a professional manner and in compliance with established time frame.
Participate as a member of a multidisciplinary team.
Ensure goals of the infant, toddler, and family are met as stated in the Individual Family Service Plan.
Provide consultation and home programs for the families/caregivers to facilitate goals as identified in the Individual Family Service Plan.
Makes referrals to and maintains contact with other agencies, organizations or providers. Coordinates services with a variety of providers and family members.
Maintains complete and accurate case management records. Develops case plans and performs general office duties, as needed.
Attends various staff training or other meetings, as required.
Performs other duties of a comparable level/type, as assigned.
