Physical Therapist – Birth to Three
The purpose of the Physical Therapist – Birth to Three is to evaluate, coordinate, and provide early intervention home based services to children with developmental delays and their families referred to the Sauk County Birth to Three program for the Sauk County Department of Human Services. The Physical Therapist – Birth to Three Early Interventionist will provide services to promote sensorimotor functions through the enhancement of musculoskeletal status, neurobehavioral organization, perceptual and motor development, cardiopulmonary status, and effective environmental adaption utilizing Primary Coach Approach to Early Intervention evidence-based practice.
Essential Duties and Responsibilities
• Provides early intervention services with child and caretaker to help meet Individualized Family Services Plan (IFSP) outcomes in natural environments utilizing Primary Coach Approach early intervention evidence-based practice.
• Participates in multi-discipline formal evaluation of developmental skills to determine program eligibility. Assists in completion of evaluation report.
• Participates in team development, review, and evaluation of Individualized Family Services Plan (IFSP).
• Serves as primary early intervention coach for families and their children (ages Birth to 3) with developmental delays, primarily children with motor delays including services directed at obtaining, interpreting, and integrating information appropriate to program planning, to prevent, alleviate or compensate for movement dysfunctions and related functional problems.
• Provides advanced coaching and guidance to B3 team and families in the area of motor development and related natural environment intervention strategies. Provides joint coaching to other primary coaches and families when there are needs for additional gross motor development support and strategies.
• Completes on-going assessment of developmental progress.
• Completes Plan of Care reports and submits to primary care physician for approval.
• Consults and coordinates with Birth to Three team members and other community partners as needed to provide the best possible early intervention services.
• Demonstrates sensitivity to cultural, linguistic, social, economic, educational, individual, and role differences among families served.
• Works as a collaborative, productive, and positive team member in promoting early intervention services within a strong teaming environment.
• Completes necessary documentation and paperwork timely to meet the federal, state, and local Birth to Three standards for documentation and insurance revenue purposes.
Additional Duties and Responsibilities
• Assist with child find, community awareness, parent/family networking activities and educational activities for families in the community.
• Assist with state file reviews and program self-assessment process.
• Participates in weekly staff meetings.
• Other duties as assigned.
Knowledge, Skills and Abilities
• Knowledge of the general principles of normal development, special education, and understanding of childhood behavior and current information in the field of education for children with special needs birth to three years of age. Advanced knowledge/education of early childhood gross motor development and related evaluations and interventions.
• Basic understanding and willingness to provide support to families regarding all areas of child development; Infant, Early Childhood, Family Mental Health; and Trauma Informed Care strategies.
• Knowledge and understanding of infants and toddlers who have delays and/or disabilities, of child development and of family structure/dynamics.
• Knowledge of Individuals with Disabilities Education Act (IDEA), Part C, and Wisconsin’s Birth to 3 Program (DHS 90).
• Effective organizational, time management, and oral/written communication skills. Strong ability to collaborate with others.
• Ability to work independently and as part of an early intervention team. Takes direction from supervisor and mentors; functions well with regular supervision.
• Ability to handle simple to moderate complex family dynamics.
• Perform evaluations of infants and toddlers who may be eligible for services.
• Work directly with families and early intervention team members in achieving the outcomes of the IFSP through implementing, monitoring, and reviewing the strategies of the child’s IFSP.
• Be available and responsive to families and team members by maintaining flexibility, confidentiality, and punctuality.
• Adhere to Birth to Three Program timelines and work expectations.
• Perform all other duties and responsibilities in certification areas as directed by supervisor.
• Explain, demonstrate, and clarify to others established policies, procedures, and standards.
• Communicate verbally and in writing with department personnel, medical care providers, educators, school personnel, children, and families.
• Adding, subtracting, multiplying, dividing, and calculating percentages, fractions, and decimals.
• Maintain all relevant certifications and training requirements.
• Compare, count, differentiate, measure, sort, classify, compute, tabulate, and/or categorize data and/or information.
• Assemble, copy, record, and transcribe data and/or information.
• Knowledge of using Microsoft Word, Excel, and electronic medical records to complete necessary paperwork and tracking dates to maintain timelines for Birth to Three clients.
• Analyze data and information using established criteria to define consequences and consider and select alternatives.
• Apply functional and situational reasoning by exercising judgement, decisiveness, and creativity in everyday routine and non-routine situations.
• Use fine and gross motor skills to perform tasks such as operating a keyboard, photocopier, telephone, calculator, and computer printer.
• Exert light physical effort in sedentary to light work, involving lifting, carrying, pushing and pulling.
Environment
• Home, community, and office environment. Generally safe and comfortable in respect to noise and disease, with the potential for exposure to violence.
Required Working Hours
Standard working hours are Monday through Friday, 8:00 am until 4:30 pm. Please note these hours are subject to change and additional hours may be needed or required. Compensation for additional hours beyond normal working hours are subject to applicable state and federal regulations.
Education and Experience Requirements
Required: Bachelor’s, Master’s, or Clinical Doctorate degree from an accredited Physical Therapy program
License: Wisconsin Physical Therapist License
Valid driver’s license
Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered
