GENERAL FUNCTION
The Mental Health Case Manager’s primary role is to support individuals who are incarcerated with their mental health. This case manager will complete mental health screenings, and assess the needs of the inmates, develop a plan to address their needs, provide ongoing support and monitoring, and work collaboratively with community partners to improve the mental health and wellbeing of the inmate. This position is an integral part of a team who assists in successfully transitioning the inmates to resources and services in the community aimed at reducing recidivism and promoting public safety.

RESPONSIBILITIES
A. ESSENTIAL FUNCTIONS
1. Review initial screenings and inmate requests to determine who needs to be seen each day, prioritizing for acuity and time sensitive needs. Meet with inmates and utilize validated tools, such as the Brief Jail Mental Health Screen, along with personal observation and existing records to identify potential mental health concerns, risk of suicide, and potential substance withdrawal symptoms. Collaborative with jail staff, leadership, community partners, and medical providers to provide quality, effective supportive listening and case management to individuals in 1:1 sessions or group settings.
2. Assess the need for emergency hospitalizations via chapter 51.15 detentions as specified in DHS Chapter 34, Subchapter 3. Initiate emergency detentions with law enforcement and collaborate with Norwood Health Center to determine placement. Coordinate with staff on return of inmate after discharge from the hospital.
3. Follow requirements for training, clinical supervision, and documentation. In coordination with Human Services, collaborate during psychiatric hospitalizations of inmates and act as a liaison with inpatient staff, community partners, and treatment teams to best meet the needs of the individual. Complete crisis plans as needed.
4. Work cooperatively with Human Service’s Behavioral Health Division for clinical supervision and oversight needs in accordance with DHS 34 requirements. Applicants working towards specific licensure may have additional supervision opportunities to obtain needed clinical hours, including individual and group supervision. Ensure required documentation is completed appropriately and timely in the county’s electronic health record of choice.
5. Support inmates on court orders and work collaboratively with treatment providers, internal and external, to ensure the health and safety needs of the inmate are met. Refer individuals for services as appropriate. Work in partnership with the Emergency Mental Health team to ensure treatment recommendations are followed.
6. Complete documentation of all screening decisions, case assignments, and services delivered in a timely and professional manner consistent with department standards.
7. Serve as a liaison with community partners.
8. Participate in meetings and ongoing trainings as directed by Wood County Sheriff’s Department and Human Services.
9. Responsible to know and practice the Safety policies of the County. Perform all job tasks in a safe and prescribed manner.
10. Must be familiar with and exemplify the Wood County Core Values and serve with courtesy and respect in accordance with the Wood County Code of Conduct.

B. OTHER DUTIES
1. Provide training as needed for jail staff, law enforcement, community partners etc.
2. Any other duties as may be assigned.

EXPERIENCE, TRAINING, QUALIFICATIONS
A bachelor’s degree from an accredited college or university in a human services area is required. Experience working with individuals involved in the criminal justice system and with those who have mental health concerns preferred. Within first three months of employment must complete training as specified in DHS Chapter 34, Subchapter 3 and receive certificate indicating eligibility to provide Emergency Mental Health Crisis Services. Additional training will also be required as necessary.

Position requires the ability to work effectively with a variety of other people; requires ability to work and manage own time effectively. Good oral and written communication skills are vital. Candidates must possess basic everyday living skills, and ability to understand and follow directions, the ability to read and write (for reports); and the ability to interview and counsel. Successful candidates will have a demonstrated ability to maintain confidentiality and professionalism, will exhibit a positive teaming approach, and will promote cohesiveness within the unit and amongst other teams and community partners.

This position may require travel to and from off-campus site on rare occasions. Possession of a valid driver’s license, as well as a licensed and insured automobile is required, or other means of reliable transportation immediately available to individual.

Skill in using common business office machines, i.e. calculator, copy machine, fax machine, scanner, and computer, is necessary.
