Human Services Professional (Children’s Long Term Support)

We are growing our Children’s Long Term Support team in 2027 and adding two new positions!

Under the general direction of the CLTS/Birth to 3 Supervisor, assess and determine both financial and functional initial and ongoing program eligibility. Coordinate supports and services on-going to help the child maximize their independence as well as providing support to the family. Performs related duties as required.
