Job Title: Ongoing Case Manager
Hours: Regular Full-Time/40 hours per week; Monday-Friday
Pay Range: $25.77 – 27.24 per hour, based on experience; plus, longevity, if applicable
Department: Lafayette County Human Services
Reports to: Family Services Unit Manager
FLSA: Non-Exempt
Position Purpose: Provide comprehensive, ongoing case management to children and youth involved in CHIPS, JIPS, and juvenile delinquency—supporting both protective services and juvenile justice interventions. May also be assigned juvenile court intake duties, including intake screening, crisis custody decisions, and court referrals.
Core Position Responsibilities:
Case Management
• Conduct assessments using Wisconsin Youth Justice and Ongoing Standards or YASI instruments via interviews, home visits, collateral contacts.
• Create, implement, and monitor service plans in collaboration with clients, families, and stakeholders.
• Manage a full caseload of children and/or youth involved in formal or informal supervision.
• Provide direct counseling, crisis response, advocate for client needs, and lead service coordination.
• Maintain thorough records and timely documentation in eWiSACWIS; prepare court-ready reports.
• Collaborate with courts, schools, law enforcement, mental health providers, and community programs.
• Supervise family interactions.
• Engage in after-hours on-call rotations and respond to emergencies as needed.
Juvenile Court Intake (as assigned)
• Conduct 24/7 intake screenings for youth brought into custody by law enforcement.
• Evaluate petitions under Wis. Stats. Chapters 48 & 938 to determine prima facie jurisdiction and custody status.
• Interview youth, families, and collateral parties; administer statutory rights.
• Provide crisis counseling, authorize detention placements as needed.
• Make and document recommendations to District
Attorney’s Office or diversion programs.
• Schedule court hearings, testify as required before court, and collaborate with detention facilities
Investigations & Protective Services (as assigned)
• Investigate suspected child abuse/neglect cases according to state standards.
• Develop preliminary service plans.
• Arrange emergency placement, testify in court, and coordinate investigations with law enforcement, medical personnel, and schools.
Administration & Reporting
• Prepare social histories, family assessments, disposition reports, permanency plans.
• Attend staff meetings, court reviews, administrative hearings, and case reviews.
• Enter documentation in eWiSACWIS accurately and timely.
• Identify system gaps and recommend policy improvements.
Professional Development
• Participate in in-service training, staff meetings, and professional seminars.
• Maintain knowledge of state statutes, agency policies, community resources, and evidence-based practices.
Position Requirements:
Education & Experience
• Bachelor’s degree in Social Work, Psychology, Human Services, or related field plus experience in child welfare or juvenile justice; or equivalent combination.
• Wisconsin Social Worker certification preferred.
Licenses & Certification
• Valid Wisconsin Driver’s License and auto insurance coverage.
• On-call duty availability, including evenings, weekends, and holidays.
Knowledge & Skills
• Proficiency in Chapters 48 & 938, family systems theory, child development, abuse/neglect intervention.
• Skilled in assessments (YASI, CANS), service planning, and crisis intervention.
• Effective communication—oral, written, and courtroom testimony.
• Strong organizational skills and ability to manage complex caseloads.

The associate frequently is required to sit, reach with hands and arms, talk and hear.
Work Environment:
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
