Human Services Case Manager

Provide ongoing case management children and families with qualifying disabilities and Medical Assistance in Children’s Long Term Support program. Assist with resources and services to children enrolled in program. QUALIFICATIONS: Bachelor’s degree from accredited school in Social Work, human services or related field; one year of related experience with individuals with disabilities or mental illness; insurance acceptable driver’s license; Qualify under WI Caregivers Law, WI Act 27. 40 Hours/week. $26.18 – $31.76/hour.
