Human Services Case Manager

Provide ongoing case management to children with a qualifying disability to receive Children’s Long Term Services and the Children’s Community Options Program. QUALIFICATIONS: Bachelor’s degree from accredited school in Social Work, human services or related field; one year of related experience with individuals with disabilities or mental illness; insurance acceptable driver’s license; Qualify under WI Caregivers Law, WI Act 27. 40 Hours/week.
