HOME DELIVERED MEAL ASSESSOR
Temporary Clerical
Division of Aging, Disability & Behavioral Health Services

The Home Delivered Meal Assessor is responsible for evaluating and ensuring the appropriateness of meals delivered to individuals. Client interaction and assessment of needs will be a focal point in this position. The assessor will also facilitate quarterly nutrition advisory council meetings. This temporary position will report to the Manager of Aging and Disability Services and will work 20-30 hours per week.

2024 Hiring Range: $20.31 – $23.08 per hour

Position Summary and Job Duties
• Interview meal program participants and complete in-home assessments
• Communicate assessment outcomes to Home Delivered Meal Coordinator
• Make referrals to community programs, services, and resources as needed
• Participate in meetings and annual trainings
• Practice competent motivational interviewing techniques
• Support and enhance a trauma-informed care environment
• Complete annual compliance training and adhere to Human Service compliance policies and procedures
• Adhere to and promote safety as a priority in the workplace
• Perform data entry of in-home assessments
• Facilitate Nutrition Advisory Council meetings
• Perform other duties as required or assigned

Success Factors

Knowledge of:
• Community resources for elders and persons with disabilities
• Wisconsin statutes, specifically Chapters 46, 54, and 55
Skill in:
• Use of Microsoft Office Suite, including Word, Outlook, and Excel
• Oral communication
• Written communication including the ability to craft correspondence and proper documentation with minimal grammatical and syntax errors
• Organization and time management
• Motivational interviewing
• Basic internet usage
Ability to:
• Travel approximately 75% of time for home visits in Kenosha County
• Take accurate notes and details regarding assessments and family contact
• Effectively collaborate and communicate with external agencies
• Work independently, multi-task, prioritize, and meet time requirements
• Remain professional and courteous at all times
• Maintain confidential information
• Work respectfully in a diverse and inclusive environment

Job Requirements, Education, Training and Experience
Required Education and Experience
• High school diploma or GED equivalent
• One to two years of work experience in Human Services field
• Sixty credits post-secondary education, especially in a Human Services or Social Services related field of study
Preferred Education and Experience
• Associate degree in Human Services or Social Services related field of study
• Experience with elders and persons with disabilities
• Experience with individuals of varying socio-economic backgrounds
• Or any combination of education, training, and experience which provides the required knowledge, skills, and abilities
Other Job Requirements
• Valid driver’s license and acceptable driving record
• Access to a personal vehicle for use in the performance of the job

• The position involves light physical demands, such as exerting up to 25 pounds of force occasionally, and up to 10 pounds of force frequently
• Vision standards required by Division of Motor Vehicles to operate a motor vehicle
• Ability to visit families in their homes which may include stairs
