CURRENT OPPORTUNITY TO JOIN OUR TEAM:
The Wood County Human Services Department has an opening for a full-time (.97 FTE) Emergency Mental Health/Adult Protective Services (EMH/APS) Coordinator Position.
GENERAL FUNCTION
Responsible for providing state mandated legal services as indicated in the State Statutes, specifically Chapters 51 and 55. Includes being responsible for monitoring individuals under commitment or settlement agreement as part of the 51.20 mental health statutes, conducting Adult Protective Services (APS) investigations, completing annual Protective Placement Reviews (PPRs), and assisting in processing of new guardianships under Chapter 54 and, as necessary, protective placements. Provide mental health emergency crisis services as part of the Mobile Crisis Response program certified under DHS 34, Subchapter 3. Respond and assess individuals as needed in the community and on site to determine needs.

RESPONSIBILITIES
A. ESSENTIAL FUNCTIONS
1. Monitor individuals (adolescents and adults) on commitment/settlement agreements related to mental health and substance use as ordered by the Wood County courts. Refer individuals for services as appropriate, including services provided directly by Human Services staff or those services that are contracted for. Monitor cases to ensure treatment recommendations are followed. Assess need for recommitment, and if recommitment is warranted, initiate court proceedings.
2. Act as liaison between Norwood Health Care Center Admissions staff and community in completing appropriate court paperwork, crisis plans, and after care plans that best meet the needs of the individual and are also cost effective for the department. Coordinate the provision of service, monitor service delivery for quality, and maintain linkage with the court.
3. Provide direct supportive services to individuals to determine and address changing needs of the individuals. May include limited case management services to address social, recreational, medical, financial, educational or daily living skills needs.
4. Conduct Adult Protective Service investigations in compliance with State Statutes and Department Policy. Initiate court action as necessary and appropriate as a result of the investigation. Make appropriate referrals for services. Complete WITS upon completion of investigation.
5. Complete annual Protective Placement Reviews (PPRs) as mandated by State Statute for Wood County residents residing in the community or institutional settings. Participate in court proceedings if PPR is contested.
6. Assist the Corporation Counsel in processing new guardianship and/or protective placement requests.
7. Provide Emergency Mental Health Crisis Services (Mobile Crisis) as specified in DHS Chapter 34, Subchapter 3 as a first responder. Follow requirements for training, clinical supervision, and documentation.
8. Create crisis and safety plans for individuals as deemed necessary.
9. Responsible to know and practice the Safety policies of Wood County. Perform all job tasks in a safe and prescribed manner.
10. Complete documentation of all case assignments in a timely and professional manner consistent with department standards.
11. Respond to various situations with minimal information to complete assessments and make recommendations for further interventions.
B. OTHER DUTIES
1. Participate in Wood County I-Team meetings, serve as a liaison with other agencies as determined, and participate in other meetings as necessary.
2. Any other duties as may be assigned.
WAGE: $52,148.20 – $59,603.70 annually
EXPERIENCE, TRAINING, QUALIFICATIONS
A Bachelor’s Degree from an accredited college or university in a human services area is required. Licensure requirements are degree specific. Experience working with individuals with mental health/developmental disabilities/physical disabilities/elderly preferred. Experience working with court systems preferred. Within first three months of employment must complete training as specified in DHS Chapter 34, Subchapter 3 and receive certificate indicating eligibility to provide Emergency Mental Health Crisis Services. Additional training will also be required as necessary. Position requires the ability to work effectively with a variety of other persons; requires ability to work and manage own time effectively. Good oral and written communication skills are vital. Candidates must possess basic everyday living skills, and ability to understand and follow directions, the ability to read and write (for reports); and the ability to interview and counsel. Demonstrated ability to maintain confidentiality. This position may require travel to and from off-campus sites. Possession of a valid driver’s license, as well as a licensed and insured automobile is required; or other means of reliable transportation immediately available to individual.
Skill in using common business office machines, i.e. calculator, copy machine, fax machine, scanner and computer, is necessary.
