The full-time Comprehensive Community Services (CCS) Service Facilitator provides community-based supports and services to people affected by substance abuse and/or severe and persistent mental illness. The CCS Service Facilitator supports the consumer in a manner that achieves the highest possible level of independent functioning, through development of a comprehensive assessment and individualized service plan designed to support psychosocial treatment goals.

Applicant must possess a Bachelor’s Degree in Social Work, Human Services, or closely related field. Prior work experience in the service delivery to people with substance abuse and/or severe and persistent mental illness are preferred. Social Work Certification or eligibility is preferred. A valid driver’s license is required.

This position may be allowed remote work for a portion of the work schedule, depending on the needs of the position, organization, work unit requirements and employee’s preference.
