Waupaca County Human Services Department is seeking a CCS Facilitator to provide trauma-informed case management and service facilitation to adults and youth with significant mental health, substance use, or co-occurring needs. This position uses a teaming or wrap-around approach to support recovery and rehabilitation goals and works under the supervision of the Behavioral Health Manager of Wraparound Treatment Services.

The CCS Facilitator completes assessments and functional screens, develops individualized recovery and crisis plans, coordinates services, and ensures compliance with documentation and billing requirements. The role involves building collaborative teams of professionals and natural supports, participating in regular clinical supervision, and working flexible hours, including evenings, to meet consumer needs.

Qualifications:
*Bachelor’s degree in social work, psychology, counseling, or a related field; licensure preferred
*Experience working with youth and adults with mental health and AODA needs
*Knowledge of recovery-based and trauma-informed practices and Wisconsin Chapter 36
*Strong communication, documentation, and interpersonal skills
*Ability to maintain confidentiality and effective working relationships
