Position Overview:
As a CCS Case Manager, you will work directly with children, families, and adults within the Comprehensive Community Services (CCS) program. Your role will involve coordinating client assessments and treatment plans, providing case management, and supporting individuals in managing mental health and substance abuse challenges. You will be a key player in empowering clients to take charge of their recovery journey, while collaborating with other professionals to ensure clients receive holistic, integrated care.
Key Responsibilities:
• Client Assessment & Treatment Planning: Develop and implement comprehensive assessments and treatment plans for individuals with mental health and/or substance abuse challenges. Help clients manage and recover from their conditions while promoting self-reliance and empowerment.
• Case Management & Coordination: Provide case management to individuals across all age groups, connecting clients to appropriate resources, and coordinating care with other service providers. Lead regular team meetings to discuss client progress and needs.
• Documentation & Reporting: Record detailed progress notes for every client interaction (phone, face-to-face, or group sessions) and ensure all paperwork is completed in a timely manner for billing and WATTS reviews.
• Crisis Intervention: Provide crisis intervention when necessary, assessing potential risks and collaborating with the crisis team to ensure the safety of the client and others.
• Ongoing Support: Assist clients with daily living activities, including tasks like grocery shopping, home repairs, and maintaining the least restrictive living environment possible.
• Team Collaboration: Work closely with therapists, counselors, and psychiatrists to ensure client needs are being met, and attend regular supervision meetings.
What We’re Looking For:
• Education & Experience:
o Waushara County values education and experience and considers both when reviewing applicants. In some cases, WI State Statute may require certifications, licenses and/or Associate, Bachelor or Master level degrees to perform in some roles. This position requires the following at a minimum: Associates
o 1,000 hours of post-degree supervised clinical experience with individuals affected by mental illness or addiction is preferred.
• Knowledge, Skills & Abilities:
o Strong communication skills and the ability to build rapport with clients from diverse backgrounds.
o Knowledge of local, state, and federal resources to assist in case management.
o Understanding of mental health and substance abuse disorders and the challenges that come with them.
o Ability to work independently and as part of a team, with a commitment to delivering compassionate, client-centered care.
• Other Requirements:
o Valid driver’s license and reliable transportation to visit clients.
o Ability to work some evenings and weekends, as well as travel to clients’ locations.
o Ability to work effectively in potentially challenging situations involving individuals who may be in crisis.

Wage & Benefits:
• Hourly Wage: $24.09 – $25.56, depending on experience.
