This position provides oversight of a continuum of Behavioral Health (BH) case management services. Program areas include; Comprehensive Community Services (CCS), Community Support Program (CSP), Coordinated Services Teams (CST) and Targeted Case Management. This position will be responsible for direct supervision of staff who facilitate and coordinate a wide range of individualized, client-centered, and trauma-informed interventions for individuals enrolled in the listed program. This position sees that services are provided in accordance with Department of Health Services Administrative Rules and Statutes.
The position reports directly to the Behavioral Health Manager. This person will also work with the BH leadership team to ensure the department provides a continuum of care for individuals suffering from Mental Health or AODA concerns. The position also provides oversite of quality assurance, productivity, compliance, and service authorizations. This position is responsible for providing truthful and accurate written and verbal communications.
Essential Job Functions
Provides direct supervision and consultation to CCS and CSP Case Managers.
Assigns cases to appropriate program and staff to best meet client needs.
Trains staff on all areas of the programs from philosophical aspect of teaming to paperwork.
Approves all authorizations for services.
Directs program operations by establishing and implementing intake and assessment procedures.
Monitors case management records.
Conducts periodic quality assurance reviews and productivity reports.
Maintains records of supervision logs, peer reviews and other quality assurance tracking.
Works with the CCS and CSP teams to develop annual goals, facilitate action steps, track progress and document/report outcomes.
Coordinates team building activities, and assists staff in professional growth in all program areas.
Facilitates weekly team meeting for each program area.
Advocates on behalf of consumers to ensure needs are met and client rights are protected, enhancing natural supports to the greatest extent possible.
Serves as liaison with JAK’s Place (the consumer resource center), the Aging and Disability Resource Center, Door County Medical Center, law enforcement, and other community services, boards and committees to address community outreach needs for vulnerable populations.
Conducts and/or participates in community awareness, education, resource development, and option exploration activities.
General Job Functions
May act as initial contact to funnel consumers to the appropriate Behavioral Health services.
Works as a member of a multidisciplinary management team to assist Director in assuring efficiency and quality in operation of the Department.
Provides back-up/on-call supervisory support to crisis staff on a rotating basis.
Assists with walk-in clients who need crisis services as needed.
Provides regular supportive contacts with consumers/families, service providers, team members and other community partners.
If needed, prepares and maintains service plans and progress notes on all assigned consumers; completes all documentation as required by the Department of Health & Human Services,
Assists youth/families and/or adult consumers in obtaining all available resources (financial, social, remedial) to enable them to reach established goals and objectives.
Completes Adult Mental Health/Substance Use Disorder Functional Screen and Children’s Long-Term Services Functional Screen Trainings to determine program eligibility as needed.
Strictly follows federal and state laws and regulations, county and departmental policies and procedures in regard to privacy, security and confidentiality of individuals’ personal and health information.
Required Training and Experience
Master’s Degree in Social Work, Psychology, Sociology or like field, with three or more years of direct service to individuals with mental health or substance use concerns. Will consider an individual with a bachelor’s degree in Social Work or like field, with a minimum of two years’ experience working in a Community Support Program.
Must meet all requirements of the Wisconsin Administrative code, DHS 63.06 and DHS 36.
Training/orientation to DHS Comprehensive Community Services and Community Support Program required within 6 months of hire.
Must complete 40 hours of Crisis Services training to meet HFS 34 Crisis Orientation within three (3) months of hire.
Possess a valid driver’s license and access to a reliable automobile.
Required to complete training and pass Department of Health Services exams to become certified as a functional screener within three (3) months of hire.
Preferred Training and Experience
Two (2) or more years of supervisory and management experience and training highly desirable.
Knowledge, Skills, and Abilities Required
Knowledge of the principles and practice of community mental health and the ability to deliver a variety of intervention services.
Knowledge of community resources.
Ability to conduct a comprehensive assessment, develop person-centered service plans and activities, develop crisis plans, and work creatively and independently.
Ability to direct and coordinate programs and work in cooperation with other managers, staff, and community partners.
Ability to listen carefully, communicate respectfully and engage in conflict resolution and problem-solving.
Ability to read, comprehend, and communicate both orally and in writing.
Ability to perform basic mathematical calculations, compare invoices to authorizations and service notes, and resolve discrepancies.
Ability to use tact and courtesy in maintaining effective working relationships with consumers, families, department employees, county supervisors, county officials, and the general public.
Ability to work independently with limited supervision.
Ability to prioritize multiple tasks in an organized, efficient manner.
