The purpose of the Community Recovery Specialist (CRS) is to assist individuals to enroll in the Sauk County Recovery Connections (SCRC) program and support those individuals in referrals to treatment of opioid and stimulant use disorder and MAT when applicable. The CRS works closely with behavioral health agencies/providers, recovery supports, and medical staff to ensure an integrated plan that is person centered and team focused and reflects both psychosocial and recovery goals of the consumers.

Essential Duties
1. Meet with consumer and others (identified by the consumer if applicable) to evaluate and assist with initial service needs.

2. Refer or conduct consumers to comprehensive mental health and substance use assessments utilizing psychometric measures (including ASAM substance use level of care assessments).
Interpret results.
Develop service and treatment recommendations.

3. Assist with navigating insurance benefits and eliminating barriers to care when able.

4. Coordinate services or specialized programs for consumers.
Acts as a therapeutic resource or team member.
Develop and implement initial recovery support plans
Transition to ongoing services
5. Intervene in crises.
Consult with other Department staff
Recommend immediate remediation including:
Residential Placement
Detoxification Placement
Respite and stabilization (ie. crisis bed)
Community Based Referrals
Facilitate and formulate crisis and safety plans, including intervention and prevention steps
6. Complete a variety of documentation such as:
Consumer referral forms
Activity logs
Progress notes
Program evaluations
GPRA Reporting

7. Develop a plan of care based on:
Consumer evaluation
Test results
Substance use assessments
Medical reports
Neuropsychological evaluations
Psychiatric/psychological evaluations

8. Consult with other community mental health providers and agency professionals regarding consumers’ needs and facilitate an integrated approach to treatment.

9. Provide education and training to stakeholders on opioid and stimulant use, community resources or services, level of care recommendations, and medication-assisted treatment.

10. Participate in program development, program supervision, grant reporting, and evaluation including Quality Assurance review and initiatives.

11. Provide public speaking and public education, as requested.

12. Maintain schedules and consumer records in the Electronic Health Record.

13. And other duties as assigned.

Required Working Hours
Standard working hours are Monday through Friday, 8:00 am until 4:30 pm. Please note these hours are subject to change and additional hours may be needed or required. Compensation for additional hours beyond normal working hours are subject to applicable state and federal regulations.

Minimum Training and Education
Required:
Bachelor’s degree in Social Work or related field
Experience and understanding of Opioid and Stimulant use and treatment
Understanding and knowledge of substance abuse treatment resources

Licenses/Certifications:
Must obtain Social Work within 6 months of hire
Valid Wisconsin State Motor Vehicle’s License

Preferred:
Substance Use credential (SAC, SAC-IT, CSAC e.g.)
1-2 years’ experience with substance use treatment
Knowledge and experience completing ASAM level of care assessments
