The purpose of this position is to conduct outreach for Public Health Sauk County (PHSC) programs with an emphasis on Maternal and Child Health (MCH). This population are families impacted by economic and racial inequities face barriers to healthcare and wellness resources that can impact health outcomes for themselves and their children.
**This is a part time – grant funded position.
**Up to 20 hours per week.
**No benefits will be offered with this position.
Essential Duties
Assists in assessing current relationships between the health department and community organizations, agencies and other stakeholders and develops plans to enhance these relationships.
Meets with medical and other service providers to inform them of services offered by PHSC that may be of benefit to their clients/patients/patrons.
Serves as a liaison between the medical community and PHSC for the purpose of exchanging resources and referrals.
Assists in the development and coordination of community and agency partnerships, initiative, programs, and events to address community health priorities. Examples of this may include staffing community events, distributing program materials; meeting with community members, organizations, medical providers, and other possible referral sources to raise awareness of Public Health Sauk County services.
Contributes to the development and implementation of the MCH outreach plan (e.g., includes measurable objectives and targets; incorporates MCH Block Grant objectives, community health improvement plan, workforce development plan, quality improvement plan, and other plans).
Documents progress toward MCH objectives and targets outlined in performance management dashboard.
Participates in community stakeholder group such as the Nurse-Family Partnership Community Advisory Board.
Serve as a member of the ICS structure if activated.
Participates in professional development.
Participates in health equity trainings and activities.
Attends staff meetings.
Other duties as assigned.

Minimum Training and Education
Required:
High school diploma or equivalent
License:
Valid Wisconsin State Motor Vehicle Operator’s License
Preferred:
Associate’s or bachelor’s degree in relevant field
Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
