In response to the increasing demand for individuals and families in need of community-based services, we are growing our team, come grow with us!
Our full-time CLTS/CCS Service Facilitator will be responsible for providing community-based case management activities to individuals across the lifespan affected by substance abuse, severe and persistent mental illness, intellectual, physical, and/or developmental disabilities. Service delivery shall be coordinated in a manner to support the consumer and their family in achieving the highest level of independent functioning within their home environment. The Service Facilitator supports the consumer by facilitating the assessment process, coordinating necessary services, documenting, and monitoring service delivery.
Applicant must possess a bachelor’s degree in Social Work or Human Services related field. Prior work experience in service delivery to people with developmental challenges, substance abuse and/or severe and persistent mental illness is preferred. A valid driver’s license is required.
This position may be allowed remote work for a portion of the work schedule, depending on the needs of the position, organization, work unit requirements and employee’s preference.
