St. Croix County has an exciting opportunity for a Case Manager / Social Worker I / Social Worker II position working with Community Support Programs (CSPs). CSPs offer coordinated professional care and treatment for adults who live with severe mental illness. Clients receive a range of services in the community. Services aim to meet a client’s unique needs, reduce symptoms, and lead to recovery. Program staff focus on these values:

ESSENTIAL FUNCTIONS:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned.
• Works to achieve involvement and participation of the client, client’s family, and client’s natural support system in resolving problems.
• Works with consumers to assess and develop goal oriented, time limited service plans to address identified needs.
• Work to protect children, elderly and at-risk adults; preserve families and individuals in the community; or to help prevent out-of-home placement.
• Assesses and evaluates prospective consumer’s immediate situation, determine appropriate response, and assure that response is made. Assess and intervene as necessary in crisis situations. Refer to and involve other community resources as needed.
• Works to achieve active involvement and participation of schools, court, health care providers, and other appropriate community resources in resolving problems.
• Advises consumers when services are either not available or inappropriate.
• Explains the consequences of non-compliance with court orders to clients who are involuntarily referred, monitors client utilization of services, and informs the court of consumer’s adherence to court orders.
• Coordinates access to services, monitors cases to ensures services are provided in an appropriate and timely manner, reviews progress, and terminates services when goals are attained.
• Teaches consumers about available community resources and how to independently gain access to needed services.
• Works to coordinate services and support consumers in achieving treatment objectives without duplication of effort and with maximum effective use of staff time and available resources.
• Maintains a complete record for all assigned cases; completes required written assessments, reports, and other related paperwork/documentation as necessary.
• Develops and maintains professional knowledge and skills to enhance the programs and services provided.
• May facilitate treatment and/or other related, indicated services with groups of clients.
• May be designated to speak to the community and other organizations to provide public education.
• Other duties as assigned.

MINIMUM QUALIFICATIONS:

EDUCATION AND EXPERIENCE REQUIREMENTS

Case Manager
• Bachelor’s degree in psychology, sociology, criminal justice, or other human service program.
• Previous practical experience preferred.
• Appointment is conditional upon successful completion of a caregiver and criminal background check.
Social Worker I
• Bachelor’s degree in social work or closely related field.
• State of Wisconsin Social Worker Certification.
• Additional experience, training, or certifications may be required for the specific Division assigned.
• Appointment is conditional upon successful completion of a caregiver and criminal background check.
Social Worker II
• Bachelor’s degree in social work or closely related field.
• State of Wisconsin Social Worker Certification.
• Additional experience, training, or certifications may be required for the specific Division assigned.
• Two (2) years’ previous experience as a Case Manager or Social Worker.
• Appointment is conditional upon successful completion of a caregiver and criminal background check.

Posting Pay Range: $55,608 – $87,477
Posting Expected Pay Range: $55,608 – $72,814
