Job Description:

The Portage House (part of the Portage County Health and Human Services department) has a full time Case Manager position opening!

What are the primary job duties?

House Coverage and Supervision, which includes:

Maintains order and safety during shift, sometimes working alone.
Monitors residents’ activities and whereabouts.
Insures house rules, policies, and residents’ commitments are being followed.
Answers phone, takes messages, and communicates to other staff regarding residents’ activities and behavior.
Remains in house during shift if working solo.
Monitors visitors during shift.
Provides first aid to residents and transports residents to the hospital (if necessary).
Provides some of the training to Relief and Night Security staff.
Assists with scheduling.
Supervises Night Security staff and some of the student interns.

Client Counseling and Supervision, which includes:

Shares client counseling responsibilities with the other Counselor. Conducts and keeps notes on weekly individual counseling sessions with each assigned client.
Facilitates some of the evening educational/training modules.
Facilitates house meetings.
Attends weekly staff meetings.
Is available to clients during shift. Supervises and interacts with clients, listens to their concerns, encourages positive behavior, confronts negative behavior.
Responds to rule or policy infractions by writing disciplinary “C-Notes”. Formulates, assigns and records disciplinary actions for clients on caseload.
Records client’s activities in log book, writes summary departure statements of assigned clients, writes other reports as necessary.
Does case planning with Department of Community Corrections (DCC) agents and other service providers. Arranges client staffing with DCC agents and others involved in clients’ program, as appropriate.
Attends supervisory sessions as scheduled.
Performs other related duties assigned by Program Director.
Attends/conducts client interviews.
Performs intake and departure procedures with clients.
Reads client files.

House Management, which includes:

Responsible for house cleanliness during shift. Assigns and monitors residents’ house chores.
Checks smoke alarms and conducts fire drills. Monitor residents’ activities during emergencies to insure safety and state code compliance.
Assigns and approves menus.
Monitors house maintenance and repair needs.

What are the minimum qualifications?

High School diploma or equivalent required. A minimum of two years post-secondary education in psychology, sociology, social work, or related human services field, or comparable training and experience required.
Group and individual counseling experience.
Any combination of education and experience that provides the equivalent knowledge, skills and abilities required to perform the essential functions of the position.
