The full-time Birth to 3 Coordinator/Case Manager conducts developmental screenings, evaluations and ongoing assessments, develops family service plans and provides coordination for developmentally challenged clients, integrates goals, objectives and outcomes to ongoing home/natural environment routines, acts as a liaison between team members and families to assist with accomplishing family outcomes and formulates written transition plans. This position also provides support, service and compliance with CCOP, CLTS and Birth to 3 programs, makes referrals to community resources and assists families with state and federal benefit programs when necessary.
Applicants must have a Bachelor’s degree in Early Childhood Special Education, Social Work, or related field and one year of experience working with families who have children with special needs. Wisconsin DPI License #808 or #809 certification with a valid teaching license is preferred. Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
