PURPOSE OF POSITION:
The purpose of this position is to serve as a para-professional to support Social Workers within the Behavioral Health Unit by primarily administering services to individuals enrolled in the Community Support Program (CSP), but also may serve those enrolled in Comprehensive Community Services (CCS), Children’s Long Term Waiver (CLTS), Birth-3, Recovery Court, IDP, adult protection and/or individuals utilizing county funded treatment that is court ordered or voluntary. This position helps ensure service plans and service delivery for each consumer is integrated, coordinated, and monitored and is designed to support the consumer in a manner that helps the consumer achieve the highest level of independent functioning. The work is performed under the direction of the Behavioral Health Services Supervisor.

ESSENTIAL FUNCTIONS:
The following duties are normal for this classification. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

Assist in developing and monitoring individualized service plans, six-month reviews, and annual updates for CSP consumers.

Meet with CSP consumers in their own homes and community to provide assistance in maintaining their independence by assisting with ADL’s and IADL’s as well as supporting their mental health.

Provide transportation coordination and/or transportation for appointments and/or to residential care.

Attend medical and psychiatric appointments with consumers as needed.

Assist in completing annual housing applications as well as Medicaid and Social Security benefits applications and reviews.

Collaborate and communicate with medical providers to schedule appointments, renew prescriptions, and obtain medical records for enrolled consumers.

Documents consumer progress. Completes required program forms, obtain necessary medical documentation, and write reports, case notes, correspondence, and other materials.

Assist in the development of Crisis Plans and APS protection plans.

Utilize Crisis Plans and Crisis Prevention Strategies in crisis situations.
Provide drug testing and data collection for the Recovery Court program.

Prepare materials for the IDP program participants, enter IDP data in to the state system and assist scheduling IDP appointments.

Comply with patient rights, grievance resolution, and consumer confidentiality requirements of the local, state, and federal governments.

Advocate on behalf of consumers. Contacts family members, friends, medical care personnel and other agencies, as assigned.

Assists in the creation and presentation of public awareness information, reports, and presentations regarding services for mental health and substance abuse recovery. Promotes inter-agency cooperation between Trempealeau County Department of Human Services and area hospitals, schools, provider, and collateral agencies.

Maintains working knowledge of Human service programs, policies, manuals, guidelines, and statutes. Attend and participate in staffing and agency meetings.

While the following tasks are necessary for the work of the unit, they are not an essential part of the purpose of this position and may also be performed by other unit members.

Answer telephone, take and relay messages, data entry and keyboarding, provide information to callers and visitors and fax/photocopy materials.

Physical Requirements

Ability to operate, maneuver and/or steer equipment and machinery requiring simple but continuous adjustments such as motor vehicle, computer terminal, typewriter, telephone, fax machine, calculator/adding machine, computer printer and photocopier.

Ability to coordinate eyes, hands, feet and limbs in performing movements requiring moderate skill, such as typing.

Ability to exert light physical effort in sedentary to light work, but which may involve some lifting, carrying, pushing, and pulling.

Ability to recognize and identify degrees of similarities or differences between characteristics of colors, shapes, sounds, and odors associated with job-related objects, materials, and tasks.

Environmental Adaptability

Ability to work under conditions which require exposure to environmental factors such as irate individuals, intimidation, odors, pets, hazardous waste, traffic hazards, and disease. This exposure may cause some discomfort and presents a risk of injury.

TRAINING, EXPERIENCE AND OTHER REQUIREMENTS:
Vocational/technical degree in human services or related field, and 2 years of experience working with individuals with mental illness and/or substance abuse, preferred in a community-based setting, or any combination of education and experience that provides equivalent knowledge, skills, and abilities. Ability to communicate effectively both verbally and in writing. Ability to engage clients establishing a good working relationship building on positive attributes. Proficient in basic computer skills. Valid motor vehicle operator’s license and/or available means of transportation required.
