Position Summary
This position to provide case management services for individuals with alcohol/drug issues, serious mental illness, and addiction issues. Other duties include providing treatment and rehabilitation services through a multi-disciplinary team along with assisting in developing and maintaining quality emergency mental health and addiction services; providing consultation to community organizations regarding addiction and behavioral health topics and services.

Essential Functions
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned.

1. Performs intake, assessments, intervention, individual, group and family counseling, outreach referral, and advocacy; follow up care; provides educational and preventive activities.
2. Assists in planning, developing, organizing and facilitating services/programs such as support and therapy groups, outpatient, prevention activities and special events.
3. Obtains information about patients through interview, case history, and observational techniques; evaluates data to identify causes of problems and to determine advisability of counseling or referral to other resources.
4. Develops program for treatment and rehabilitation of client, using knowledge of alcohol and drug abuse problems and counseling and treatment techniques.
5. Provides and/or arranges for therapeutic interventions as appropriate for patients or clients in a crisis condition.
6. Provides information, education and support family members to assist family in dealing with providing support for client.
7. Assesses and manage situations that require immediate attention including walk-in clients, clients in need of detoxification services, emergency inpatient evaluations appointments for residential services funding and emergency telephone crisis encounters.
8. Maintains accurate documentation relevant to cases, including assessments, recovery plans, relapse prevention plans, progress notes and discharge summaries.
9. Attends and arranges staffing and team meetings to ensure quality care and maximization of resources. Keep program manager apprised of consumer progress, needs, and any critical issues.
10. Provides consultation to other units within DHHS and to the community regarding mental health and addiction services.
11. May participate in county Recovery Courts.
12. Attends court hearings and provides expert testimony as required.
13. Collaborates with other sections of DHHS to ensure effective services for consumers and the linkage of the consumer to economic, health, and family support services.
14. Refers client to other support services as needed, both internal and external. Coordinates counseling efforts with mental health professionals and other health professionals.
15. Documents assessments, treatment plans, reviews, referrals and discharge planning and summaries in accordance with professional standards, agency, state and federal guidelines.
16. Bills all allowable services and submit billing as directed. Ensures state reporting system forms and data collection are completed on a timely basis.
17. Other duties as assigned.

Knowledge, Skills, and Abilities
• Knowledge of the needs of and dynamics involved with persons with addiction and/or serious mental illness.
• Knowledge and experience in utilizing the most recent version of the DSM.
• Knowledge and ability to utilize The ASAM criteria.
• Knowledge and ability to utilize The Matrix Model Treatment program.
• Knowledge of crisis intervention services and strategies and ability to work with individuals in crisis and those with challenging behaviors.
• Knowledge of addiction and integrated dual disorders treatment.
• Knowledge of community resources to assist those with a serious mental illness and addiction.
• Knowledge of administrative policies and procedures of the County.
• Knowledge of current office practices and procedures and knowledge of the operation of standard office equipment and software.
• Ability to establish and maintain accurate records of assigned activities and operations.
• Ability to interpret and implement local policies and procedures; written instructions, general correspondence; Federal, State, and local regulations.
• Skill in organizational and time management to prioritize duties to accomplish a high volume of work product while adapting to constant changes in priority.
• Ability to perform detailed work accurately and independently in compliance with stringent time limits with minimal direction and supervision.
• Ability to think quickly, maintain self-control, and adapt to stressful situations.
• Knowledge of computer software including word processing, spreadsheet and database applications consistent for this position.
• Ability to perform mathematical calculations required of this position.
• Ability to perform detailed work accurately and independently in compliance with stringent time limits with minimal direction and supervision.
• Ability to understand and effectively carry out verbal and written instructions.
• Ability to apply sound judgment and discretion in performing duties, resolving problems and interpreting policies and regulations.
• Ability to communicate detailed and often sensitive information effectively and concisely, both orally and in writing.
• Ability to handle sensitive interpersonal situations calmly and tactfully.
• Ability to maintain effective working relationships with individuals within and outside the organization.
• Ability to maintain confidentiality and discretion regarding business-related files, reports and conversations, within the provision of open records laws.
• Ability to work the allocated hours of the position and respond after hours as needed.

Required Education, Experience, Licensing, and Certifications
• Current licensure as a Substance Abuse Counselor (SAC) or SAC-IT in the State of Wisconsin.
• Three (3) years of experience working with individuals with mental illness and addiction preferred.
• Bachelor’s Degree in social worker or related field preferred.
• Must successfully pass caregiver and criminal background check.

• Work is primarily in an office setting, although work may be performed in a secured inmate setting.
