Education & Experience: Associate Degree related to human development, prevention, and/or gerontology, plus two years of experience working with people who are older and/or adults with disabilities and/or equivalent combination of education and experience. Required: High School diploma plus 5 years of experience working with people who are age sixty and older and/or adults with disabilities
License/Certifications: Valid Wisconsin Driver’s License acceptable driving record, and have access to private transportation for work-related duties. AED/CPR Certification
Or any equivalent combination of education, training, and experience, which provides the necessary knowledge, skills, and abilities.
Essential Duties
JOB SUMMARY:
The role of the Independent Living Support Program Specialist is to receive referrals, enroll participants into the pilot program, create a care plan of services for older adults or adults with physical, intellectual, or other developmental disabilities, follow up with the participants and create a transition plan when the pilot program will be ending.

MAJOR RESPONSIBILITY:
Support the planning, development, and implementation of Independent Living Support Pilot Program for older adults and adults with disabilities to retain or improve functioning and to delay or prevent the need for comprehensive long-term care services.

Meet one on one with potential participants to determine eligibility for the Independent Living Support Program Pilot.

Assist participants with identifying, finding, and securing resources to assist the person to remain independent in their home.

Develop a care plan with the participant and follow up with participants on their on-going needs on a regular basis throughout the grant period.

Maintain and report data as determined by the Department of Health Services and ADRC.

Complete required program and participant records, documentation, using appropriate narrative, data entry, and/or spreadsheet format.

Provide transitional support to participants following the completion of the service plan and grant allocation, and/or during the last 3-4 months of the pilot project.

Support the planning, development, implementation, and evaluation of program supports that impact the independence and support needs for those wanting to remain living in their homes without going onto Long Term Medicaid funding programs.

Monitor care plans and work on sustainability of support to delay or prevent individuals from going onto long term support programs.

Report on utilization to the ADRC Finance Department and I & A Coordinator monthly for approval.

Network with community organizations and providers for the purpose of developing partnerships to ensure sustainability of Independent Living Support Program grant programming within the county for ADRC target populations.

Promote ADRC services when presenting and educating the community and enrollees about programs and resources.

Provide education and training about the Independent Living Pilot Program and services to ADRC staff to ensure information can be shared with customers.

Attend and participate in staff development programs, dementia and caregiving educational events, ADRC in-service training, staff meetings, and professional seminars as requested by Information and Assistance Coordinator or Director.

Comply with applicable federal and state laws, administrative rules, established agency procedures and accepted professional standards.

Participate in the planning and goal setting efforts of ADRC.

Participate in the work setting as a team player.

Has a “Duty to Report” during a community emergency/disaster according to the ADRC Emergency Preparedness Plan.

Perform other related duties as required or assigned

Minimum Qualifications Required
MINIMUM QUALIFICATIONS REQUIRED:
Education and Experience:

Knowledge of computer and applications and usage, including all Microsoft office programs.
