Job Title: Public Health Nurse
Pay Range: $32.26 – 34.10 per hour, based on experience; plus, longevity, if applicable
Hours: Regular Full-Time/40 hours per week; Monday-Friday
Department: Lafayette County Health Department
Reports to: Public Health Officer/Director
FLSA: Non-Exempt

Position Summary: Under the direction of the Public Health Officer/Director, the Public Health Nurse carries out the requirements for a Level II Health Department, including the foundational capabilities of Public Health which are: Assessment and Surveillance, Community Partnership Development, Equity, Organizational Competencies, Policy Development and Support, Accountability and Performance Management, Emergency Preparedness and Response and Communications.

Essential Functions:
• Prevention and control of communicable diseases through education and intervention using epidemiological principles.
• Plans, organizes, implements and evaluates immunization programs.
• Participate in ongoing community assessment program planning and implementation.
• Participates in emergency preparedness planning, response and recovery.
• Supervises volunteers/student nurses and independent contractors in health department activities.
• Provides public health nursing assessments and care for individuals and families through home, office or telephone visits.
• Participate in professional development programs and ongoing education to maintain current knowledge of public health issues and current nursing practices.
• Coordinates assigned grant activities, as directed by the Director.
• Assists in the collection of data for statistical information/analysis.
• Participates in health-related community partnerships and networks locally and statewide.
• Provides screening, referral and follow up for the purpose of identifying health needs.
• Collaborates on environmental health and human health hazard situations.
• Provides health counseling and education in a variety of community settings in person, over the phone and/or electronically.
• Plans, organizes, implements, and evaluates chronic disease prevention activities through the Health Department.
• Case management activities, for example, linking clients to care.
• Perform examinations of water samples.
• Participate in community health-related education and promotion.
• Performs other activities as assigned.

Other Duties/Responsibilities:
• May need to work after hours and some weekends.
• May be asked to provide oversight to agency when Director is not available.
• Must treat all information with the utmost of confidentiality.

Minimum Qualifications:
Education and Experience:
• Bachelor of Science Degree in Nursing (BSN) from an accredited program.
• One-two years of experience performing nursing duties related to the position preferred.

Licenses/Certifications:
• Current license to practice professional nursing in Wisconsin during employment.
• Valid driver’s license and proof of auto insurance coverage.
• BLS/CPR certification within 30 days of hire.

Knowledge, Skills and Abilities:
• Knowledge of the theory, techniques, practices and standards of professional nursing.
• Knowledge of public health programs such as public health nursing, environmental health, and communicable disease.
• Knowledge of State Statutes associated with the provision of public health services.
• Ability to work independently in meeting a variety of public health regulations and requirements.
• Ability to maintain records and to prepare reports.
• Ability to establish and maintain effective working relationships with the public, community groups, school personnel, and state and regional public health agencies.
• Ability to prioritize projects and time.
• Skill in the administration of immunizations. Knowledge in the use of various health-related screening tools.
• Present self to public in a professional manner utilizing excellent communication skills.
• Knowledge of HIPAA/Confidentiality laws for Public Health.

Other requirements:
• Emergency Management Training (ID-700a, ICS 100b, ICS 200b, ICS 800b, ICS 300, ICS 400) within 6 months of hire.
• Completion of Tuberculosis (TB) screening at time of employment and a TB risk assessment annually during employment.
• Proof of current immunizations at time of employment.

Physical Demands:
• Work is performed both in an office setting and in an outdoor environment involving steep topography, barnyards, temperature and weather extremes, and potential exposure to agricultural chemicals.
• Ability to exert moderate physical effort, typically involving some combination of stooping, kneeling, crouching and crawling, lifting, carrying, pushing and pulling.
• Office work is performed from a sedentary position; however, considerable movement is required to retrieve files, information or assist customers.
• Ability to coordinate eyes, hands, feet and limbs in performing skilled movements such as keyboard use.
• Ability to operate computer keyboard, calculator, fax machine, telephone and computer printer.
• Ability to hear normal conversation and communicate in a clear manner as well as work with frequent interruptions.
• Application of tact and understanding to resolve conflict or trouble-shoot problem situations is frequently required.
• Willingness and ability to accept assignments in various cultural, physical, behavioral and environmental settings.
• Possible exposure to blood-borne pathogens, other communicable diseases, noise and odors.
