Educational Requirements:
• Maintains a Valid Register Nurse license in the State of Wisconsin
• Two or more years’ experience is preferred
• BLS, ACLS, PALS, and NRP Certified
• No Exclusion from participation in Medicare, Medicaid or other Federal healthcare programs.

Professional Functions of the Cardiopulmonary Director in regards to rehab patients:
• Ability to provide leadership and direction to the patient care team.
• Ability to perform age specific physical, psychological, and social assessments; utilizing that information to develop a plan of care to include individualized treatment strategies.
• Ability to manage, organize, and provide direct patient care; incorporating clinical decision making and prioritizing of activities.
• Ability to educate the patient, appropriate family members, and other healthcare team members; to include the incorporation of health maintenance, disease process, and discharge planning.
• Ability to organize, within the specific work period, those aspects of care for which he/she is accountable, and coordinate the patient care with other health team members.
• Ability to document patient intake on Electronic Health Record (EHR).
• Ability to complete daily age appropriate assessments on all patients to include:
o Respiratory System
o Cardiovascular System
o Neurological System
o Gastrointestinal System
o Musculoskeletal System
o Pain Management
• Ability to complete patient blood sugar monitoring as appropriate.
• Ability to provide appropriate patient education upon discharge.
• Ability to enter quality data statistics.
• Ability to perform initial new participant assessments; assisting with new participant orientations and the development of a plan of care, working as a team with other staff members.
• Ability to ensure that all required documentation is in EHR prior to a new participant admissions into Cardiac or Pulmonary Rehab including:
o Signed Physician Referral
o Signed Orders
o Recent EKG
o Consent Form
o Patient History
o Patient Assessment
o Patient Care Plan
• Ability to supervise exercise sessions, monitor participants for inappropriate responses to exercise.
• Ability to perform blood pressure checks on all participants pre and post exercise, watching for abnormal responses on telemetry.
• Ability to assess participants’ pre, during and post exercise, making appropriate referrals and/or recommendations to the participant, other staff members, and provider.
• Ability to monitor blood pressure, rate of exertion, and telemetry, before, during and after exercise.
• Ability to perform glucometer testing on participants with diabetes.
• Ability to report immediately, any significant participant condition (dysrhythmias, abnormal blood pressure responses, change in participants’ status etc. to the physician supervising the program.
• Ability to initiate ACLS measures and/or emergency care according to hospital policy.
• Ability to chart and document initial assessment and daily participants’ information and program.
• Ability to document exercise session in flow sheet with progress notes.
• Ability to schedule initial assessment visit with patient, schedule exercise and education sessions, with all patients.
• Ability to ensure the availability of an in-house physician to assume responsibility for the Cardiac Rehab program.
• Ability to ensure emergency supplies and equipment for proper function are available on a regular basis.
• Ability to check all equipment is in working order so there is no disruption of care.
• Ability to check that crash cart and other supplies are on hand, and defibrillator is tested before each session.
• Ability to report any significant event and/or situation to the Department Manager or Administration.
• Ability to perform Life Support measures for adult and pediatric patients

• Full range of body motion and dexterity to allow for the significant handling, transferring, pushing/pulling, and lifting of materials and equipment.
