Community Health Planner

Under the direction of the Portage County Health and Human Services-Division of Public Health Manager, this position serves as a public health strategist within a nationally Public Health Accreditation Board (PHAB) accredited local health department. The health planner position works in collaboration with community groups, other agencies/departments, and resources to develop and implement community health services or policies by conducting assessments, capacity building, planning, implementation, and evaluation strategies. This position supports goals and objectives for identified health focus areas of the Community Health Improvement Plan for Portage County and Calls to Action for the Portage County LIFE Report. A good understanding of the essential services and core functions of public health is expected.
