Job Summary
Responsible for the coordination of the Brown County system of services for children 0-3 with developmental delays and/or disabilities and their families. Educates the public, referral network and collaborating partners about the program, as defined by IDEA Part C and DHS 90. Works with Social Service Aide to manage referral process and coordinates entire Birth to 3 service delivery system. Conducts developmental assessment of children, brief assessment and explanation of program with family. With team, assists to complete an evaluation to determine program eligibility and a routines-based family assessment. Monitors and ensures timelines are met. Assists families with referrals to desired community resources and transition process to future supports at age 3.

Essential Duties
Accepts referrals and conducts initial home visits with families. Educates parents of their child and family early intervention rights and privacy laws. Provides written prior notice and obtains consent for screening and evaluation. Notifies family of system of payments and calculates parental cost share amount. Determines child’s developmental abilities and needs through the use of developmental screening and assessment tools, parent report, professional observation and review of medical reports. With team members, performs an evaluation to determine if the child is eligible for Birth to 3 services. With team members, assists with conducting a family assessment to identify family interests, routines, activities, strengths and concerns. Completes Targeted Case Management documentation. Communicates all necessary data and authorizations with program supervisor. Develops and maintains family IFSP (Individualized Family Service Plan). Coordinates reviews of established IFSP every 6 months or as needed. Coordinates and ensures timely transition plans are established and participate in determination of appropriate options as children transition out of Birth to 3.

Attend weekly team meetings with all Birth -3 team members and support practice of Primary Coaching and Teaming.

Participates in child find activities to locate, identify and evaluate potentially eligible children from the community.

Ensures adherence to all timelines and program requirements as per DHS90 and IDEA Part C.

Works to promote that all services are provided utilizing evidence-based practices to promote positive outcomes for children under the age of three and their families as required by DHS90 and IDEA Part C. Fully understand, utilize and support others in their practice of the Primary Coach Approach to Teaming model.

Maintains collaborative contacts with social workers, schools, medical professionals and collaborative agency staff, emphasizing a team approach.

Maintains informed referral network and network of collaborating partners. Knowledge of community needs, resources and opportunities. Provides requested training and in-services to community programs, universities and businesses about child development, resources and Birth to Three. Participates in community coalitions or committees relevant to the Birth to 3 population.

Maintains case records and documentation as required by DHS90.

Assists in the resolution of parent complaints and refer unresolved complaints for fair hearings.

Identifies and appoints surrogate parents as necessary.

Initiates referrals to community options, Children’s Long-Term Support, Katie Beckett and other supplemental resource programs as needed.

Maintains the confidential nature of client and business information and HIPAA compliance.

Complies with all local, state and federal laws governing clients and the rendering of services.

Attends staff meetings, attends in-service and outside agency training sessions, including 5 hours of early intervention training per year to meet DHS90 requirements.

NON-ESSENTIAL DUTIES:

Minimum Qualifications Required
Education and Experience:

Bachelor’s degree in social work, teaching or related human service field, with an emphasis on infant and toddlers with delays and /or disabilities. One year supervised experience working with families with special needs required, five years preferred.

Licenses and Certifications:

Valid Wisconsin Driver’s License

Knowledge, Skills & Abilities
Possess expertise in the assessment of typical and atypical child growth and development, specific to age span birth to age 3.

Ability to effectively engage and interview parents/caregivers for successful completion of family assessment and initial development of the Individualized Family Service Plan.

Knowledge of birth to three early intervention laws and guidelines, including DHS90, Public Law 99.457, Special Education Law 89.313, and IDEA Part C.

Knowledge of Medical Assistance (MA) case management requirements and procedures.

Knowledge of accessing private insurance.

Knowledge of human growth and development, with pediatric emphasis desired.

Knowledge of and ability to utilize a computer, web-based programs and the required software.

Ability to do public speaking.

Ability to assess families, determine the need for services and develop Individualized Family Service Plans to support families.

Ability to communicate effectively, both orally and in writing.

Ability to establish and maintain effective working relationships with staff and the public.

Ability to work the required hours of the position.

PHYSICAL DEMANDS:

Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.

Extended periods of sitting; intermittent standing, walking and driving.

Using hand(s)/feet for repetitive single grasping, fine manipulation, pushing and pulling, and operating controls.

Occasional bending, twisting, squatting, climbing, reaching, and grappling.

Communicating orally in a clear manner.

Distinguishing sounds at various frequencies and volumes.

Distinguishing people or objects at varied distances under a variety of light conditions.
