PURPOSE OF POSITION:
The purpose of this position is to promote health and wellness and provide skilled public health nursing services to persons in the community. The work is performed under the supervision of the Director/Health Officer.

ESSENTIAL FUNCTIONS:
Participate in response efforts to identify, prevent, minimize, and contain adverse health events and conditions resulting from communicable diseases; food, water and vector-borne outbreaks; chronic diseases; environmental hazards; injuries; and health disparities.

Deliver presentations to community, academic groups, and/or policy makers (i.e. Board of Health) on public health related issues.

Collaborate with other community and state partners (i.e. universities, businesses, etc.) in the planning and implementation of evidence-based public health programs and nursing practices.

Participate in, and at times, lead local and regional coalition efforts.

Assist with research, data collection, writing and evaluation of contracts and grants. Manage assigned grants and/or contracts. Ensure objectives are met and year-end reports are completed.

Contribute to the development and implementation of policies, programs and services, including program goals and objectives. Evaluate the effects of existing policies, programs and services, and make recommendations for program enhancements. Participate in Quality Improvement projects and use Performance Management system to improve department processes and impacts.

Participate in the creation and implementation of the organizational Strategic Plan and Community Health Improvement Plan.

Performs in home, environmental lead consultations and education.

Participate in accessing, collecting, analyzing, using, maintaining and disseminating community health-related data and information; assess needs pertinent to the health of individuals, families, populations or communities; make evidence-based decisions using data; apply ethical principles on the use of data.

Maintain confidentiality of medical information, records and data pursuant to County policies and procedures the State of Wisconsin and Federal HIPPA regulations.

Adhere to applicable guidance from the state and federal health departments relating to environmental health, immunizations and control of communicable diseases, including enforcement of isolation and quarantine measures.

Adhere to applicable Occupational Safety and Health Administration standards, such as those concerning exposure to blood borne pathogens, toxic substances, airborne pathogens or exposure to other hazards during routine assignments or during public health emergencies.

Coordinate activities among and between other governmental agencies that enforce laws and regulations that protect the public’s health.

Promote and provide population-based health services including:

Setting up and operating public health nursing clinics (i.e.-foot care, immunizations, vision screening, hemoglobin screening, blood lead screening, child physical exams, pregnancy testing, TB skin test, and fluoride varnish application).
Working with, and at times, providing program management for programs such as Communicable Disease, Immunizations, Lead, Maternal Child Health, Oral Health, Preparedness, and Tuberculosis.
Conducting communicable disease and sexually transmitted disease investigations. Performing follow-up counseling and referral as appropriate.
Administering injections/immunizations, collect patient test samples, treat wounds, perform health assessments, perform capillary blood draws, and other skilled nursing care as appropriate.
Traveling to patients’ homes to provide nursing care, client evaluation, referral and case management
Arranging and participating in multi-disciplinary meetings or medical appointments as indicated for the individual, family, or group
Advocating on behalf of vulnerable individuals and populations
Participating in the evaluation of health care services to ensure people are informed of and assisted with the use of available health department and community programs/services.
Provide leadership in the orientation and guidance of new staff members, volunteers and students.

Assume responsibility for own professional growth and development by pursuing education and participating in professional committees and work groups.

Maintain an understanding of the diversity of Trempealeau County residents; recognize the influence of diversity in policies, programs and services.

KNOWLEDGE, SKILLS, AND ABILITIES:
Thorough knowledge of professional nursing techniques, current Nurse Practice Act, related WI State Statutes, and Administrative Code.

Proficiency in computer skills for charting, tracking and reporting.

General knowledge of community health nursing: communicable disease, childhood lead, public health preparedness and the ability to provide immunizations.

Ability to incorporate ethical standards of practice into all interactions.

General knowledge of community resources and their roles and functions.

Ability to work and communicate with patients of a variety of socio-economic levels and to provide culturally competent care. Ability to follow precise oral and written medical instructions and complete charting paperwork accurately and in a timely manner.

Ability to work and communicate orally and in writing with physicians, Public Health personnel, state agency personnel, patients’ families, human services personnel, pharmacists, clinic/hospital personnel, lab personnel and jailers.

Ability to work independently with minimal supervision, effectively prioritizes responsibilities, and makes sound nursing judgments.

Ability to analyze data and information using established criteria, in order to determine consequences and to identify and select alternatives. Ability to compare, count, differentiate, measure, copy, record and transcribe data and information. Ability to classify, compute, tabulate, and categorize data.

Ability to counsel, treat and mediate, and/or provide first line supervision. Ability to persuade, convince, and train others. Ability to advise and provide interpretation regarding the application of policies, procedures and standards to specific situations.

Ability to utilize a variety of advisory and design data and information such as patient records, lab reports, physician orders, plan of care, maps, flow charts, statistical reports, personnel policies, employee performance evaluations, time study sheets, policies, nursing procedure manuals, medical textbooks and computer software operating manuals.

Ability to calculate percentages, fractions, decimals, volumes, ratios and present values. Ability to interpret basic descriptive statistical reports.

Ability to use functional reasoning and apply rational judgment in performing diversified work activities.

Ability to exercise the judgment, decisiveness and creativity required in situations involving the evaluation of information against sensory and/or judgmental criteria, as opposed to criteria which are clearly measurable.

TRAINING, EXPERIENCE AND OTHER REQUIREMENTS:
Bachelor’s degree in Nursing from an accredited program required and two years nursing experience preferred, or any combination of education and/or experience that provides equivalent knowledge, skills and abilities. Current CPR certification must be maintained. A valid motor vehicle operator’s license, and Registered Nurse in the State of Wisconsin required. Experience in maternal child health, immunizations, communicable disease investigation, coalition building, and program management preferred. Experience in a community health or governmental public health setting preferred. Bilingual (English/Spanish), or experience working with an interpreter preferred.
