The Portage County Health Care Center is hiring a full time RN Manager to join their team!

Summary of Position:

Administrative Functions: Assists the Director of Nursing in the planning, organizing, development and supervision of the day to day functions of the Nursing Department. Works in accordance with current Federal, State and local standards, guidelines and regulations that govern the facility and as may be required by the Director of Nursing to ensure that the highest degree of quality is maintained. Oversees the day to day nursing activities performed for residents.

Physician/Nurse Practitioner Related Functions: Reviews resident orders with prescribing physician when necessary. Contacts physician when necessary regarding problems, changing conditions and specific requests. Ensure physician orders are interpreted, transcribed and followed correctly.

Planning and coordination of Nursing Services/Dementia.: Works with Social Services Staff during pre-admission screening to assess and determine appropriateness for placement in the facility. Provides the Director of Nursing with information relative to the needs of the dementia care residents and the department’s ability to meet those needs. Works closely with unit nursing staff and activities staff with resident dementia specific programming; activities and multi-sensory room.

Supervision of Resident Care: Make daily rounds on Units. Review and monitor clinical records and resident care plans to ensure that they are informative and descriptive of the nursing care being provided, and reflect the resident’s response to the care. Assist and verify nursing staff assessments, MDS validity and monitoring of resident’s conditions. Audit Nursing documentation for thoroughness and completeness. Administer professional services in the care of residents according to the physician’s orders, nursing judgment and facility policies. Delegates professional nursing tasks only which are commensurate with education and demonstrated abilities. Provide direction and supervision in such tasks.

What are the minimum qualifications?

Associate degree in nursing.
Registration as a nurse in the State of Wisconsin.
Experience supervising and directing the work of others.
Experience in restorative nursing, psychiatric nursing, medical nursing or geriatric nursing.
Experience working on a memory impairment unit preferred.
Ability to successfully pass a caregiver background check.
