Coordinated Services Facilitator
DUTIES: Performs intermediate skilled human support work by providing direct support services to assigned clients in behavioral health and long-term support programs in the community including the Comprehensive Community Support Program (CCS), Coordinated Services Team (CST), Community Support Program (CSP), Children’s Long-Term Support Medicaid Waiver Program (CLTS), and the Behavioral Health Clinic; providing service delivery, accessing and transporting individuals to community services, employment programs, and appointments for assigned clients., and related work as apparent or assigned. Work is performed under the limited supervision of the Coordinated Services Manager and or Supervisor.
REQUIRED QUALIFICATIONS: Requires a Bachelor’s degree in vocational rehabilitation, psychology, social work, or health-related field; and one year of experience working with specified target groups (elderly, disabled, mentally ill, etc.) may be required. For Coordinated Services Facilitators assigned to Comprehensive Community Services, Community Support Program, and Children’s Long-Term Support Waiver Services, certification as a Long Term Care Functional Screener is required. Requires working knowledge of the principles of assessment, intake, and access to services. Maintains knowledge of current programs, policies, procedures, ordinances, and laws. Applicants must pass background checks conducted by the Wisconsin Department of Justice and the Wisconsin Department of Health & Family Services, and pre-employment drug and other screenings conducted as required by County policy.
Wage: $22.29 per hour, depending on Certification. Includes a generous benefits package.
