Job Description:

Provide support to individuals with dementia and their caregivers; educate individuals and families about options, assist with making connections to programs and services through both private and public funding, provide cognitive screening for early detection of cognitive change, assist with physician connections for diagnosis and providing evidence-based programs; engage in outreach and collaborative efforts with internal and external providers in order to increase community awareness related to dementia and advocating for consumer needs. Qualifications: Bachelor’s degree from accredited school in health or human services field; one year of experience working in dementia field, i.e. direct care, caregiver support, support group facilitation, residential care, homecare; understand types of Dementia and its effects on individuals; insurance acceptable driver’s license; qualification under the Wisconsin Caregivers Law, Wisconsin Act 27. 40 Hours/Week. $21.82 – $24.94/hour.
