Social Worker – Psych / Intensive In-Home Services
Portage County Health & Human Services is hiring a full-time Social Worker to support the Psych / Intensive In-home Services Section.
Summary of Position:
This position participates in a team approach to provide in-home therapy services for severely emotionally disabled (SED) children and families. It provides behavioral skill development, case management, and consultation services to individuals and their families as directed by the lead in-home therapist.
What are the primary job duties?
Provides behavioral skill development, case management, and consultation services, which includes;
Implements and monitors individualized treatment plans for intensive in-home therapy clients as directed by lead in-home therapist.
Provides coordination to community agencies for SED children and families as needed.
Records clinical treatment notes and progress notes.
Prepares and presents client cases for staffing with primary therapist and physicians.
Provides client transportation when appropriate.
What are the minimum qualifications?
Bachelor degree in social work or a related human service field required.
One year experience within the last seven years working with the mentally ill required, experience with children’s mental health preferred.
Certification or eligible for certification as a Social Worker in Wisconsin preferred.
