Summary: The Social Worker/Case Manager position is responsible for identifying and assessing complex individual, family and community needs and strengths, selecting case work frequency, group methods or community resources and implementing plans to protect vulnerable people of all ages, promote independence and prevent abuse or neglect.
** Special Requirements:
Waiver Services Dual Services Social Worker/Case Manager:
To be considered for the position of Waiver Services Dual Services Social Worker/Case Manager, candidates must meet at least one (1) of the following standards:
• Certified or licensed Social Worker under Wis. Stat. § 457.08 and have a minimum of one year of experience working with children and/or youth with disabilities.
• The skills and knowledge typically acquired through a course of study and practice experience that meets requirements for state certification or licensure as a social worker and one year of experience with the target group(s) for which they are employed – children’s long-term support
• A course of study leading to a Bachelor of Arts or Bachelor of Science degree in a health or human services related field and one year of experience working with person s of specific target group(s) for which they are employed – children’s long-term support
• An associate degree in human services related field and two (2) years of experience working with persons of the specific target group(s) for which they are employed – children’s long-term support
• Any combination of training in a health or human services related field and experience in long term care case management and/or case management for individuals with disabilities or special needs that totals four (4) years. Examples include but are not limited to: one year of coursework or training in human services related field and three (3) years of experience in long-term care case management, or four (4) years of experience in long-term support case management.

Duties/Responsibilities
A Social Worker/Case Manager may be assigned duties in any of the following departments: Children and Families, Behavioral Health and Public Health.
Essential Duties:
Coordinate consumer intake:
• Receive referrals, conduct interviews, and investigate to identify and assess needs, risk for recidivism, or need for intervention/service;
• Develop, write, implement and monitor strength-based individual and family service plans to correct/eliminate identified needs;
• Determine appropriate resources and referrals to community-based care and services; and
• Develop and implement safety plans to address vulnerabilities
**Some mandated service programs require 24/7coverage with on-call requirements
Determine and implement intervention services:
• Assess risks to self and others, assess safety to self, others, and the community;
• Intervene in crisis situations, including possible removal from home and/or placed in alternate care;
• Coordinate and secure emergency services; and
• Negotiate and resolve conflicts.
Provide client case management:
• Collaborate and consult with others (professional staff and community agencies) regarding consumers, community services or programs;
• Open and maintain consumer case records and files according to established standards and timelines,
• Prepare necessary correspondence, required narratives, eligibility reviews and other incoming/outgoing communication about the consumer;
• Provide information regarding case openings, closings, and denials;
• Provide testimony or written recommendations in county or district court or in administrative appeal hearings regarding consumer cases;
• Advise consumers of resources, rights and responsibilities under programs or services; and
• Develop, coordinate, and deliver program information to the general public, special interest groups and professional organizations.
Perform other duties as assigned.
Marginal Duties and Responsibilities: Other duties as assigned and general office back-up and support.
Supervisory Responsibilities:
Limited to Social Worker III:
1. Designated Lead Worker appointed by hiring authority and/or
2. Supervises interns in accredited Social Work programs.

Case Manager I:
1. Bachelor’s Degree Human Service or related field.
2. A combination of education and experience may be considered.
3. Must have a valid driver’s license.
Social Worker I:
1. Bachelor’s Degree in Social Work or related Human Service field.
2. Must be certified as a Social Worker by the State of Wisconsin.
3. Must have valid driver’s license.
Social Worker II:
1. Bachelor’s Degree in Social Work or related Human Service field.
2. Must be certified as a Social Worker by the State of Wisconsin.
3. Two year’s paid Social Work experience in a county human service organization.
4. Must have valid driver’s license.
Social Worker III:
1. Bachelor’s or Master’s Degree in Social Work (BSW/MSW only), certified or licensed Social Worker by the State of Wisconsin with three year’s paid Social Work experience in a county human service organization or
2. Bachelor’s or Master’s Degree and certified as a Social Worker by the State of Wisconsin with five year’s paid Social Work experience in a county human service organization
3. Must have valid driver’s license.

Minimum Skills Assessment Requirements: Interviewing and assessment skills and the ability to formulate evidence-based recommendations are required. Exceptional oral and written communication skills are necessary.
Certificates/Licenses/Registrations: Social Worker I, II, and III must have a minimum certification as a Social Worker by the State of Wisconsin.
