Our Behavioral Health unit is looking for energetic, flexible and knowledgeable individuals to join our team today!

Under supervision of Behavioral Health Supervisor provides referral, case management, and direct services to adults with chronic mental illness and/or substance use disorders in residential settings and the community.
Essential Duties
Screens and evaluates individuals to determine appropriate services based on individual client needs.

Monitors Chapter 51 commitments and settlement agreements.

Provides crisis services, including crisis assessment, intervention, and follow up.

Develops and implements service plans that provide appropriate services to the client and their families.

Provides primary case management for assigned clients, including assessment, service planning, monitoring, and evaluation.

Provides direct community based services to specific client populations.

Participates in discharge planning from inpatient or nursing home placements.

Provides consultation to human service agencies.

Provides community education to the public and various agencies.

Completes and maintains necessary documentation and reports according to applicable requirements, codes and policies.

Accepts requests and referrals for unit services.

Recruits volunteers by utilizing media, speaking engagements and other methods.

Provides supervision and other services to adults living in substitute care placements.

Makes collateral contacts with medical, legal, mental health and other human services agencies and organizations; provides information and referral services.

Performs related functions as assigned.

Minimum Qualifications Required
Education and Experience:
Bachelor’s or Master’s degree in social work or related human services field; experience in social service agency dependent upon the particular position preferred.

Licenses and Certification:
Valid Wisconsin Driver’s License
State Social Worker Certification preferred

Knowledge, Skills & Abilities
Knowledge of principles, methods and practices of social work.

Knowledge of current social and economic problems and the effect of these problems on families and
individuals.

Knowledge of laws, regulations and practices pertaining to federal and state public welfare programs.

Knowledge of federal, state and local resources, their organization and the ways in which these resources can
assist individuals and families.

Knowledge of living conditions, values and behavior of the ethnic and subcultural groups served by the
agency.

Ability to work flexible hours, including on-call

Ability to attend staff and other meetings as needed

Ability to maintain confidentiality

Ability to provide environmental and supportive social services to individuals and/or families.

Ability to diagnose and provide appropriate treatment services.

Ability to plan and organize work to achieve objectives.

Ability to relate to people in an unprejudiced and understanding manner.

Ability to establish and maintain working relationships within agency and the community.

Ability to communicate effectively both orally and in writing.

Ability to participate in and appropriately use available supervision.

Ability to work the required hours of the position.
