Job Summary
Provides assessment and crisis intervention services; responsible for the investigation of persons referred under the Adult at Risk and the Elder Abuse Neglect reporting systems; determines needs and develops short term intervention and support plans; manages services; coordinates and petitions the court for Guardianship Services; and refers consumers to other appropriate service providers.
Essential Duties
Conducts investigations on referrals of abuse, neglect, and exploitation of vulnerable, disabled adults and elder persons.

Reviews and assesses service referrals to determine need for guardianship and/or protective placement within the standards and parameters established by the state and county department.

Determines financial eligibility for guardianship and other services.

Petitions the court and assists the court in establishing legal guardianship and protective placement for adult and elderly consumers who are incompetent.

Develops and implements short term service plans that provide appropriate services and achieve targeted outcomes based on individual needs.

Consults with local county and private service agencies in regard to provision of investigation and guardianship services for adult and elderly consumers at risk.

Provides annual reviews of protective placements.

Coordinates process of annual reviews with court-appointed guardians ad litem.

Prepares for and coordinates with other service units and agencies for court proceedings regarding annual reviews of protective placements.

Prepares comprehensive evaluations in relation to protective placements.

Provides information to the public and agencies in regard to the Protective Service Program and regarding services under Chapters 46.90, 51, 54 and 55 Wisconsin Statutes.

Collaborates on interdisciplinary teams to assure comprehensive system response, program and service development on behalf of adults and elders at risk.

Assists in developing, maintaining, and revising procedures and formats for the legal documentation involved in protective services and placement.

Assists in developing program changes as required by protective-service laws.

Prepares individual cases assigned for court or legal action.

Intervenes in crisis situations on behalf of adults and elders at risk.

Provides case management for assigned consumers, including assessment, treatment planning, monitoring, and evaluations under Medicaid and other relevant program rules.

Maintains the confidential nature of client and business information.

Complies with all local, state, and federal laws governing clients and the rendering of services.

Attends staff meetings, attends in-service and outside agency training sessions.

Completes and maintains timely and accurate documentation according to applicable requirements, codes and policies.

Performs related functions as assigned.
