Purpose of Position
The purpose of this position is to provide services through the Aging & Disability Resource Center including: information and assistance, long term care options counseling, resource development and maintenance, functional screens, marketing and public education. The work is performed in accordance with applicable rules, regulations and laws and under the direction of the ADRC Supervisor.

Essential Functions
To perform this job successfully, an individual must be able to perform each essential function satisfactorily and in a timely manner. The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required or assigned.

Assess the needs of the caller, evaluate appropriate resources, indicate organizations capable of meeting those needs, help callers for whom services are unavailable by locating alternate resources and link caller to needed services.

Make referrals on behalf of the caller/customer to other community services.

Provide long-term care options counseling and follow-up.

Maintain quality assurance by seeking feedback about caller/customer satisfaction. Assist in the development and implementation of the quality assurance/quality improvement plan.

Perform functional screen application for potential Family Care and/IRIS participants. Assist potential participants will all aspects of enrollment requirements.

Participate in community education, marketing, and prevention campaigns.

Ensure the protection of customer rights and advocate for necessary services.

Maintain functional screen certification as required by Department of Health Services.

Maintain knowledge of eligibility requirements for community service and funding sources.

Participate in federal time reporting requirements.

Inform long-term support staff and other agency staff about the resources available through the Resource Center and in the community and coordinate interagency meetings for the Resource Center.

Maintain working knowledge of human service programs, policies, manuals, guidelines and statutes. Attend training and participate in unit and agency staff meetings.

Serve on area or state committees or boards related to social service programs or service delivery.
Maintain technical manuals. Take and relay messages. Photocopy printed materials.

Knowledge, Skills, and Abilities
Ability to analyze data and information using established criteria in order to determine consequences and to identify and select alternatives. Ability to compare, count, differentiate, measure, copy, record and transcribe data and information. Ability to classify, compute, tabulate and categorize data.

Ability to counsel, treat and mediate, and/or provide first line supervision. Ability to persuade, convince and train others. Ability to advise and provide interpretation regarding the application of policies, procedures and standards to specific situations.

Ability to utilize a variety of advisory data and information such as billings statements, budgets, expenditure reports, assessment reports, resource reference materials, program policy/procedure manuals, directories, medical equipment catalog, computer software operating manuals, statutes, procedures, guidelines and non-routine correspondence.

Ability to communicate orally and in writing with clients, department personnel, medical care providers, state agency personnel and the general public.

Ability to add, subtract, multiply, divide and calculate percentages, fractions and decimals.

Ability to use functional reasoning and apply rational judgment in performing diversified work activities.

Ability to exercise the judgment, decisiveness and creativity required in situations involving the evaluation of information against sensory and/or judgmental criteria as opposed to criteria which are clearly measurable.

Training, Experience and Other Requirements
Bachelor’s Degree in Social Work, two years social work and case management experience preferred, or any combination of education and experience that provides equivalent knowledge, skills and abilities. Experience in developing community alternatives for elderly and disabled persons and knowledge of long term care options and resources preferred. Wisconsin Social Work Certification required or must receive Wisconsin Certification within two years of hire. Valid motor vehicle operator’s licensed and/or available means of transportation required.

Ability to coordinate eyes, hands, feet and limbs in performing movements requiring moderate skill, such as typing.

Ability to exert light physical effort in sedentary to light work, but which may involve some lifting, carrying, pushing and pulling.

Ability to recognize and identify degrees of similarities or differences between characteristics of colors, shapes, sounds and odors associated with job-related objects, materials and tasks.

Environment / Work Conditions
Ability to work under conditions which require exposure to environmental factors such as irate individuals, intimidation, pets, hazardous waste, traffic hazards and disease. This exposure may cause some discomfort and presents a risk of injury.
