Our CPS unit is looking for passionate, caring and driven individuals to join our team now!

Under supervision, assists professional staff in non-technical services by conducting home and community visits, transporting clients, making observations, documenting activities, and performing routine clerical and related tasks as assigned.

Full-time position (37.5 hrs per week) with scheduling between 8am-8pm Monday to Friday with occasional weekend hours.

Essential Duties
Interviews individuals and family members to compile information on social, educational, medical, criminal, institutional, and other history.
Schedules meetings, visits with, observes, supervises, and assists consumers with personal and daily living needs.
Offers information on, assists with applications, and refers individuals to community agencies and services for assistance.
Reports progress, activity, and observations of individuals or families as they relate to the case plan.
Gathers information and prepares reports about consumer status and progress.
Provides transportation and delivery assistance as needed.
Documents activities and provides routine clerical support to professional staff including filing, completion of reports, and other documentation; files legal documents and serves legal notices; assists in data entry on departmental automation and documentation systems; assists in monitoring and maintaining inventories.
Participates in and contributes to team meetings.
Performs other duties as assigned.
Minimum Qualifications Required
High school graduation and prior work or volunteer experience in a human services organization required. Associate or bachelor’s degree in a human services related field preferred.

Knowledge of household management, daily living and personal care competencies.
Ability to assess possible problems during visitations and take appropriate actions.

PHYSICAL DEMANDS:
Lifting 25 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.
Lifting and transporting small children.
Frequent standing, sitting, walking and driving.
Using hand(s)/feet for repetitive single grasping, fine manipulation, pushing and pulling, and operating controls.
Occasional bending, twisting, squatting and grappling; frequent climbing and reaching.
