PURPOSE AND SUMMARY:
Provides a full range of intensive in-home or office-based psychosocial rehabilitative and treatment services to consumers. Works collaboratively within team or individually.
ESSENTIAL FUNCTIONS:
Performs intake evaluation duties by screening and assessing emotionally and behaviorally impaired children and their families who are referred for services, and effectively engage these families to move them into change process to improve their family functioning and overall home environment.
In conjunction with team members, develops and carries out plans for overall rehabilitation, treatment, and care for assigned children and families to address consumer and community safety concerns. Provides case consultations and performs case coordination services with other involved agencies and service providers including participation in multidisciplinary staffing meetings for assigned families.
Documents services provided within the consumer’s case record in a manner spelled out in the clinic’s policies and procedures, and completes various forms required for insurance authorization, disability determination, program eligibility, service tracking.
REQUIRED EDUCATION, EXPERIENCE, TRAINING, AND CERTIFICATION:
Master’s degree in social work, psychology, counseling, or a closely related human service field.
Qualified and licensed as a Wisconsin Licensed Clinical Social Worker, Professional Counselor, Psychologist, or Marriage and Family Therapist. Dual Substance Abuse Counselor certification preferred.
At least 3,000 hours of supervised experience.
Minimum of three years of supervised experience in family systems-based psychotherapy services.
Requires qualification under Wisconsin Caregivers Law, Wisconsin Act 27.
Preferably knowledge of a county’s child welfare system and the impact the expectations of that system have on the delivery of therapy treatment services to like consumers of both child welfare and behavioral health systems.
