Human Services Case Manager

Provide ongoing case management to adults and children with qualifying disability to receive Supportive Home Care services, Medical Assistance Personal Care services or Children’s Long Term services; provide services to adults with serious persistent mental illness through the Community Options Program. QUALIFICATIONS: Bachelor’s degree from accredited school in Social Work, human services or related field; one year of related experience with individuals with disabilities or mental illness; insurance acceptable driver’s license; Qualify under WI Caregivers Law, WI Act 27. 40 Hours/week. 2024 Pay Range: $24.28 – $30.91. 2025 Pay Range: $25.55 – $30.98.
