Coordinated Service Teams (CST) Case Manager

This full-time case management position will provide services with families in a collaborative partnership who are voluntarily participating in the Coordinated Services Team (CST) initiative as outlined in §46.56. CST Initiatives develop a comprehensive, individualized system of care for children with complex behavioral health needs, utilizing a wraparound model of care. This position reviews all referrals and makes direct contact with families referred to CST. This position will work to develop community resources to provide support to children and families. The coordinator will develop and foster cooperation between individuals, community agencies and other systems of care to meet the identified needs of the family. This position is rooted in the Core Values of CST, including but not limited to being family centered, consumer involved and strengths-based.
