CCS Service Facilitator
Sawyer County Health and Human Services is accepting applications for a fulltime Comprehensive Community Services (CCS) Service Facilitator. CCS is a voluntary, community based statewide program administered by counties that provides psychosocial rehabilitation for program participants who need ongoing services for a mental illness, substance use disorder, or a dual diagnosis beyond occasional outpatient care, but less than the intensive care provided in an inpatient setting. Under the direct supervision of the Behavior Health Clinic Supervisor/CCS Services Director, the CCS Service Facilitator will provide a wide range of client-centered case management services and works with participants in reaching their recovery goals. Services provided may include the coordination of mental and/ or physical health care, assistance with housing, money management, medication management, coordination of substance abuse treatment, coordination of vocational and/ or education support, and assistance with activities of daily living. As with all positions at our agency, it is the duty of the individual in this position to fully support the mission, vision and values of the agency, to work in a coordinated manner with other employees, and to demonstrate professional and personal confidence in the service delivery model and approaches taken by the organization. The Service Facilitator is expected to integrate trauma informed care while serving our communities.
Sawyer County (population 17,000) is situated within the pristine woods and waters of northwestern Wisconsin. Sawyer County Health and Human Services is located in the county seat of Hayward. Hayward is the home of world-class events such as the American Birkebeiner and the Lumberjack World Championships.
Qualifications for this position include a bachelor’s degree in social work, psychology, or closely related field. Current Comprehensive Community Services (CCS) certification and be qualified under DHS Chapter 36.10 (2)(g) 1 to 21 to perform the overall responsibility for service facilitation (within 2-weeks of hire). Minimum of 2 years’ relevant experience working with individual with substance abuse/or severe and persistent mental health challenges or comparable experience that provides relevant knowledge and skills. Experience providing home and community based services. Knowledge of Sawyer County’s child and adult community resources, including evidence based mental health and AODA treatment services. Commitment to strength-based, trauma informed care, recovery-focused programming and practice concepts. Must pass a background check and urine screen. Valid motor vehicle operator’s license and proof of insurance. Proficient computer skills, specifically in current versions of Microsoft Word, Excel, Outlook and PowerPoint, and willingness to learn new skills as needed or required.
