Case Manager- Community Services

Overview/Purpose of Position:
Clark County is seeking an organized, energetic individual to provide case management services to adults with severe and persistent mental illness and substance use disorders who are enrolled in the Community Support Program (CSP). This is a full-time position (40 hours per week) with a competitive wage and excellent benefits.
The individual in this position will determine functional eligibility, conduct comprehensive assessments, and develop and monitor individual service plans while providing treatment and rehabilitation services through a multi-disciplinary approach. They will work collaboratively with other professionals, including but not limited to, contracted providers, clinic/hospital staff, social workers, law enforcement agencies, etc.

Requirements:
A Bachelor’s Degree in Social Work or a Human Services related field is required along with at least two years of case management experience. A minimum of two years of experience working with individuals with mental illness and/or substance use disorders is preferred. An equivalent combination of education and experience will be considered. Applicants must have two of more years of experience providing services in community-based settings as well as a valid Wisconsin driver’s license and auto insurance as required by County policy. Applicants must pass background checks conducted by the Wisconsin Department of Justice and the Wisconsin Department of Health Services

Benefits:
Pay range for this position is $26.00 – $32.96, based on experience and qualifications, plus a competitive benefit package.
