Wage: $52,148.20 – $65,245.70/annually
CURRENT OPPORTUNITY TO JOIN OUR TEAM:
The Wood County Human Services Department has an immediate opening for a Birth to Three Service Coordinator (Certified or Uncertified).
GENERAL FUNCTION:
The Service Coordinator applies the Wisconsin Birth to Three Program guiding principles, and supports the primary coach approach to teaming in natural environments to families of children under the age of three with developmental delays.
ESSENTIAL DUTIES AND RESPONSIBILITIES: All duties pertain to Certified or Uncertified (Unless noted)
1. Provides expertise on navigating the early intervention system, using family-centered practices, linking families to community resources, fostering strong family-professional partnerships, and facilitating and documenting the early intervention process.
2. Coordinate the delivery of all services across agency lines and connect children and their parents with early intervention and other services needed as identified in the IFSP.
3. Demonstrate and embrace an understanding of the primary coach approach to teaming in natural environments and serve as an advocate for the effectiveness of this approach.
(Certified only) 4. Complete comprehensive evaluations and assessments on the overall development of children by utilizing the Developmental Assessment of Young Children, Second Edition (DAYC-2) or other standardized evaluations/assessments as assigned, on all children who are in need of an evaluation to help determine eligibility into the Birth to Three Program.
5. Be an active team member who participates in all scheduled team meetings and staff development opportunities/trainings.
6. Establish and maintain effective relationships with young children, parents, co-workers, the general public, and other agencies
7. Communicate clearly and effectively in writing when preparing written reports for child/family evaluations/assessments, Individualized Family Service Plans (IFSP), and case notes in Streamline Healthcare Solutions LLC (SmartCare) including information on client strengths, outcomes/goals, transitions, and educational plans.
8. Work collaboratively on various levels with the Birth to Three team, parents, day care providers, and other caregivers to identify and develop strategies to accomplish identified outcomes.
9. Complete required program reports in accordance with agency and statutory guidelines within agency time frames.
10. Incorporate into daily practice the Birth to Three Program mandates and best practices, as well as Individuals with Disabilities Education Act (IDEA) Part C and how early intervention services are to be provided requiring the use of evidence-base practices that happen in real-life settings.
11. Perform all duties in compliance with standards set out in Medicaid Targeted Case Management regulations.
B. OTHER DUTIES
1. Actively participate in the development and implementation of quality improvement projects.
2. Provide consultation and education services to clients, organizations, and community agencies as request.
3. Attend in-service trainings as appropriate to meet licensing requirements, and to continue personal and professional growth.
4. Any other duties as assigned.
MINIMUM TRAINING AND EXPERIENCE TO PERFORM ESSENTIAL JOB FUNCTIONS:
(Uncertified) A Bachelor’s Degree in a human services field is required with at least one year experience working with families who have special needs.
(Certified) A Bachelor’s Degree in Social Work with a certification under Wis. Stat. ch. 457 is preferred with at least one year experience working with families who have special needs.
2. Traveling is a requirement for this position. Possession of a valid driver’s license, as well as a licensed and insured automobile is required; or other means of reliable transportation immediately available to individual.
3. Basic office equipment used: computer, printers, telephone, copy machine, fax machine and calculator is required, as is operational familiarity with Microsoft Word and Excel software and Streamline Healthcare Solutions LLC (SmartCare) software.
